Provider Status definition

Provider Status means an individual or entity granted approval to develop continuing education program offerings without prior approval from the board for each program offered. All continuing education offerings shall utilize the same provider authorization number assigned by the board. A licensed individual shall review each continuing education program as noted on the provider application or most recent renewal application on file with the board.
Provider Status. The Provider shall maintain its status as a school-approved by the Commonwealth of Virginia (or provider’s state) State Board of Education and shall notify the Buyer promptly in the event that such approval is withdrawn, revoked, or threatened to be withdrawn or revoked. Such withdrawal or revocation shall immediately terminate this Contract without financial obligation on the part of the Buyer to pay the Provider’s invoices subsequent to the loss of the approved status. The Provider shall maintain that all teachers hold a valid teaching license issued by the Virginia Department of Education (VDOE) and are teaching within their endorsement area. A certified copy of such licenses shall be provided to the Buyer. Attendance: The Provider shall maintain individual monthly attendance records which shall be submitted with monthly invoices to both the Buyer and the Case Manager for SCPS. If a student has an unexcused absence for three or more consecutive school days and/or for a period of more than eight days in one month, the Provider shall investigate the reason for the absence. Such absence, regardless of reasons, shall be reported immediately to the Buyer and SCPS. The Buyer shall be notified either in writing, verbally, or telephonically of unexcused absences exceeding 72 hours, by a representative of the Provider in a timely manner, but not later than eight hours after the third consecutive absence. SCPS staff shall also be notified either in writing, verbally, or telephonically of absences, regardless of the reasons, for purposes of planning for continued stay. Should the Provider fail to notify the Buyer of the unauthorized absences within the timeframes allotted in this Addendum, the Buyer is not obligated for payment of unauthorized absences. The Provider shall notify the Buyer at such time when/if the student returns to placement within 72 hours. If a child is authorized for a service in an acute care setting or setting in which the child is unable to participate in his/her special education placement, that placement will be held for the child no more than 72 hours without prior written approval from the Buyer. Such absence shall be reported as stipulated in the Contract or this addendum.
