Special Incident Report definition

Special Incident Report. A report of an incident, emergency, or special occurrence involving YOUTH, drivers, and/or administrative staff that requires documentation and follow-up actions or any vehicular accident occurring during service delivery to YOUTH. Special incidents would include, but are not limited to, any incident of unusual, aggressive, or high-risk behavior by a YOUTH, a medical emergency, AWOL YOUTH, lost property, or injury to a YOUTH, driver, and/or administrative staff member.
Special Incident Report is the documentation prepared by vendor staff or long- term health care facility staff detailing a special incident and provided to Regional Center and appropriate law enforcement authorities.
