Documentation Standards. The Contractor shall ensure day treatment intensive and day rehabilitation documentation meets the documentation standards described in Attachment 9 of this exhibit. The documentation shall include the date(s) of service, signature of the person providing the service (or electronic equivalent), the person’s type of professional degree, licensure or job title, date of signature and the total number of minutes/hours the beneficiary actually attended the program. For day treatment intensive these standards include daily progress notes on activities and a weekly clinical summary reviewed and signed by a physician, a licensed/waivered/registered psychologist, clinical social worker, or marriage and family therapist, or a registered nurse who is either staff to the day treatment intensive program or the person directing the services.
Documentation Standards. The Contractor shall set standards and implement processes that will support understanding of, and compliance with, documentation standards set forth in this section and any standards set by the Contractor. The Contractor may monitor performance so that the documentation of care provided will satisfy the requirements set forth below. The documentation standards for beneficiary care are minimum standards to support claims for the delivery of specialty mental health services. All standards shall be addressed in the beneficiary record; however, there is no requirement that the records have a specific document or section addressing these topics.
Documentation Standards. All documentation that supports findings and determinations made under this Agreement must be consistent with 36 CFR 800.11.
Documentation Standards. The Contractor may be requested to provide products or services in accordance with rules, regulations, OMB orders, standards and documentation as specified by the agency’s order.

Written documentation of activities prescribed by Stipulation II of this PA shall conform to the Secretary of the Interior’s Standards and Guidelines for Archaeology and Historic Preservation (48 FR 44716-44740), as well as to applicable standards and guidelines established by the SHPO.
Documentation Standards. A. All documentation that supports the findings and determinations made under this Agreement shall be consistent with 36 CFR 800.11 and shall be in accordance with the Authority’s requirements and its subsequent revisions or editions and with attachments to this Agreement. Documentation shall be submitted to the Authority and prepared by QIs who, at a minimum, meet the Secretary of the Interior’s Professional Qualifications Standards (48 FR 44738-44739) (Appendix A to 36 CFR Part 61). The Authority shall review the documentation for adequacy, and transmit all documentation cited herein as stipulated by this Agreement.
B. All documentation prepared under this Agreement shall be kept on file at the Authority and the FRA and made available to the public without the inclusion of culturally sensitive information that may jeopardize confidentiality as stipulated by this Agreement, consistent with applicable confidentiality requirements and Federal records management requirements.
Documentation Standards. Reporting on and documenting the actions cited in this Agreement shall conform to every reasonable extent with the Secretary of the Interior’s Standards and Guidelines for Archeology and Historic Preservation (48 Fed. Reg. 44716-40 dated September 29, 1983), as well as the BLM 8100 Manual; the Archaeological Resource Management Reports (ARMR): Recommended Contents and Format (ARMR Guidelines) for the Preparation and Review of Archaeological Reports; and any specific and applicable county or local requirements or report formats. This list represents the guidelines available during development of this Agreement. Should the guidelines be updated after the execution of this Agreement, the latest versions will take precedent. In the event that any guidelines are modified in the future to conflict with this Agreement, the BLM shall notify all Consulting Parties and will consult to determine how this Agreement should be revised, if necessary, pursuant to Stipulation X. All records and all reports will be filed with the appropriate California Historic Resource Information Center. In areas of potential overlap with lands managed by other federal and state agencies (e.g. USDA Forest Service, National Park Service, Department of Defense installations, Tribal lands, California State Lands Commission, etc.), BLM will file GIS data and site forms with the appropriate agency in addition to the above mentioned Information Centers.
Documentation Standards. 1. Written documentation of inventory, evaluations, findings of effect, and treatment prescribed per this Agreement will conform to the Secretary of the Interior’s Standards and Guidelines for Archeology and Historic Preservation (48 FR 44716–44740), as well as to applicable standards and guidelines established by the SHPO and the HPMP. All documentation that supports findings and determinations made under this Agreement shall be consistent with 36 CFR Part 800.11. Documentation prepared by local or state agencies or their consultants in support of such findings shall be submitted to the Corps for review and approval by the Corps District Archaeologist or appropriately supervised Corps staff.
Documentation Standards. The Contractor shall ensure day treatment intensive and day rehabilitation documentation meets the documentation requirements in BHIN 22-019.
Documentation Standards. A. The Contractor shall implement and comply with documentation standards as set forth in guidance issued by the Department in BHIN 22-019.
