These preventive care services are covered either under Medicare Part B or under Standard SHARP. Any of the preventive services listed below which are not covered by Medicare Part B will be reimbursed under Standard SHARP at no cost to the covered individual. Standard SHARP will not pay for any of the listed preventive care services which are eligible for coverage under Medicare Part B, nor will it pay for services listed below that exceed the frequency specified. If a frequency for the service is not specified, one such service per calendar year will be covered. Standard SHARP means the plan of benefit options described in the Standard SHARP-Ex 2017 document. Instructions for Completing the SHARP Form The age 65 Eligible Retiree and/or Eligible Spouse must be enrolled in Medicare.

Examples of Standard SHARP in a sentence

  • The HRA and Earned Credit is the monthly amount that is made available to assist an Eligible Retiree with the costs of the Standard SHARP DVH Option if selected and provide the HRA amount for the SHARP- Ex Option.

  • An Eligible Dependent described above shall cease to be eligible for benefits under Standard SHARP when he/she attains age 26.

  • Dependent Children Eligibility A dependent child of an Eligible Retiree or Eligible Spouse may be eligible for coverage under Standard SHARP.

  • All Medicare-eligible individuals may only choose from the Standard SHARP Option.

  • Changes to Standard SHARP The Adventist Retirement Board reserves the right to amend Standard SHARP based on financial considerations or other unanticipated circumstances such as changes to Medicare.

  • Please refer to the Standard SHARP booklet for coverage and enrollment.

  • This termination rule applies even if the person otherwise meets the requirements for a Standard SHARP open enrollment period described above in the Limits for Enrollment Changes section.

  • If eligible for an Earned Credit, an Eligible Retiree or Eligible Spouse who selects benefits under both Standard SHARP and the Pre-Medicare SHARP will receive two Earned Credits, one for Standard SHARP and another Earned Credit for Pre-Medicare SHARP.

  • Standard SHARP Earned Credit may only be used for Standard SHARP.

  • Ambient air means that portion of the atmosphere, external to buildings, to which the general public has access.

  • Cleaner means a product designed and labeled primarily to remove soil or other contaminants from surfaces.

  • Glass cleaner means a cleaning product designed primarily for cleaning surfaces made of glass. Glass cleaner does not include products designed solely for the purpose of cleaning optical materials used in eyeglasses, photographic equipment, scientific equipment and photocopying machines.

  • Gasohol means a blended fuel composed of gasoline and fuel grade ethanol.

  • Contractor Selection of Leave Trees means crop and leave trees are unmarked and will be selected by the Contractor.

  • Bathroom and tile cleaner means a product designed or labeled to clean tile or surfaces in bathrooms. “Bathroom and Tile Cleaner” does not include “Toilet/Urinal Care Product.”

  • CalRecycle means the Department of Resources Recycling and Recovery.

  • Life insurance producer means any person licensed in this state as a resident or nonresident insurance producer who has received qualification or authority for life insurance coverage or a life line of coverage pursuant to chapter 522B.

  • Tight-fitting facepiece means a respiratory inlet covering that forms a complete seal with the face.

  • Soda fountain means a place especially equipped

  • Clean ballast means the ballast in a tank which since oil was last carried therein, has been so cleaned that effluent therefrom if it were discharged from a ship which is stationary into clean calm water on a clear day would not produce visible traces of oil on the surface of the water or on adjoining shorelines or cause a sludge or emulsion to be deposited beneath the surface of the water or upon adjoining shorelines. If the ballast is discharged through an oil discharge monitoring and control system approved by the Administration, evidence based on such a system to the effect that the oil content of the effluent did not exceed 15 parts per million shall be determinative that the ballast was clean, notwithstanding the presence of visible traces.

  • Pneumatic tire means every tire in which compressed air is designed to support the load.

  • Septage means the liquid and solid material pumped from a septic tank, cesspool, or similar domestic sewage treatment system, or from a holding tank, when the system is cleaned or maintained.

  • Ambient air quality standard means an established concentration, exposure time, and frequency of occurrence of air contaminant(s) in the ambient air which shall not be exceeded.

  • Clean wood means natural wood which has not been painted, varnished or coated with a similar material, has not been pressure treated with preservatives and does not contain resins or glues as in plywood or other composite wood products.

  • Dental hygienist means an individual who has fulfilled the educational requirements and is a graduate of an accredited dental hygiene program and who has passed an examination and has been issued a license by the Board and who is authorized to practice dental hygiene as hereinafter defined;

  • Bathing means washing oneself by sponge bath; or in either a tub or shower, including the task of getting into or out of the tub or shower.

  • Air freshener means any consumer product including, but not limited to, sprays, wicks, powders, and crystals, designed for the purpose of masking odors or freshening, cleaning, scenting, or deodorizing the air. Air fresheners do not include products that are used on the human body, products that function primarily as cleaning products, disinfectant products claiming to deodorize by killing germs on surfaces, or institutional or industrial disinfectants when offered for sale solely through institutional and industrial channels of distribution. Air fresheners do include spray disinfectants and other products that are expressly represented for use as air fresheners, except institutional and industrial disinfectants when offered for sale through institutional and industrial channels of distribution. To determine whether a product is an air freshener, all verbal and visual representations regarding product use on the label or packaging and in the product's literature and advertising may be considered. The presence of, and representations about, a product's fragrance and ability to deodorize (resulting from surface application) shall not constitute a claim of air freshening.

  • Automobile Headliner Adhesive means an aerosol adhesive designed to bond together layers in motor vehicle headliners.

  • Double Phase Aerosol Air Freshener means an aerosol air freshener with the liquid contents in two or more distinct phases that requires the product container be shaken before use to mix the phases, producing an emulsion.

  • Carpet and upholstery cleaner means a cleaning product designed for the purpose of eliminating dirt and stains on rugs, carpeting, and the interior of motor vehicles and/or on household furniture or objects upholstered or covered with fabrics such as wool, cotton, nylon or other synthetic fabrics. “Carpet and Upholstery Cleaner” includes, but is not limited to, products that make fabric protectant claims. “Carpet and Upholstery Cleaner” does not include “General Purpose Cleaners”, “Spot Removers”, vinyl or leather cleaners, dry cleaning fluids, or products designed exclusively for use at industrial facilities engaged in furniture or carpet manufacturing.

  • Municipal corporation means, in general terms, a status conferred upon a local government unit, by state law giving the unit certain autonomous operating authority such as the power of taxation, power of eminent domain, police power and regulatory power, and includes a joint economic development district or joint economic development zone that levies an income tax under section 715.691, 715.70, 715.71, or 715.74 of the Ohio Revised Code.

  • Faux finishing coating means a coating labeled and formulated as a stain or a glaze to create artistic effects including, but not limited to, dirt, old age, smoke damage, and simulated marble and wood grain.

  • Transverse plane means a vertical plane perpendicular to the median longitudinal plane of the vehicle;

  • Electrical cleaner means a product labeled to remove heavy soils such as grease, grime, or oil from electrical equipment, including, but not limited to, electric motors, armatures, relays, electric panels, or generators. Electrical Cleaner does not include “General Purpose Cleaner,” “General Purpose Degreaser,” “Dusting Aid,” “Electronic Cleaner,” “Energized Electrical Cleaner,” “Pressurized Gas Duster,” “Engine Degreaser,” “Anti-Static Product,” or products designed to clean the casings or housings of electrical equipment.

  • trade effluent means any liquid either with or without particles of matter in suspension therein which is wholly or in part produced in the course of any trade or industry carried on at the trade premises, and in relation to any trade premises means any such liquid as aforesaid which is so produced in the course of any trade or industry carried on at those premises, but does not include domestic sewage;
