Treatment Agreement definition

Treatment Agreement means a document outlining the responsibilities and expectations of the OBOT provider and the patient that is signed and dated by the patient.
Treatment Agreement means a written agreement, signed by the practitioner and the patient (or the patient's proxy), which shall become part of the patient's medical record.
