Welfare Program Document means a written arrangement, including any contract between an Employer and an insurance company, health maintenance organization (“HMO”), administrative service organization (“ASO”) or other similar organization to provide benefits, a plan document or other instrument under which a Welfare Program is established and operated.ASSISTANCE WITH YOUR QUESTIONS

However, such Welfare Program Document does contain certain terms of the Plan. Any reference to a Welfare Program Documentalso refers to any amendment, rider, endorsement, exhibit or attachment thereto.

  • Except as otherwise provided in a Welfare Program Document for the life insurance benefit, no benefit, right or interest of any Participant, Dependent or Beneficiary under a Welfare Program may be assigned without the Plan Administrator’s written consent on behalf of the Plan.
