Funded Program definition

Funded Program means a set of services paid through a particular funding source for the benefit of a specific beneficiary (e.g., Medi- Cal/Healthy Families or Non-Medi-Cal/Non-Healthy Families). The Funded Program Amount is the basis for the provisional payment to the Contractor per Paragraph E of the Financial Exhibit A of the LAC-DMH LE Agreement. A Funded Program is made up of one or more Subprograms;
Funded Program is defined as a program of a not-for-profit organization or collaborative partnership meeting certification standards, receiving discretionary funding from or through UWLS to provide local services that assist in achieving significant impact in the established impact areas. This agreement governs programs receiving funding through UWLS’s Community Impact process. With a realization that both UWLS and the Funded Program organization face increasingly difficult challenges and increasing demands, the Agreement presents opportunities for the organizations to work together cooperatively rather than separately, to achieve the overall goal of building a better community. Within the Spirit of Agreement, UWLS and Funded Programs agree: • To provide/support local services that are oriented to the achievement of measurable outcomes; • To assist each other in achieving community change through the community impact areas; • To actively engage in community conversations/roundtables to explore and expand community impact initiatives; • To communicate with each other any issue or problem that potentially could have a negative impact on the working relationship; and • To actively and enthusiastically promote the partnership between UWLS and the organization of the Funded Program, and vice versa, to the public in a positive manner. Recognizing that this agreement does constitute a contractual relationship between the United Way of Lake and Sumter Counties, Inc. and the organization of a Funded Programs, the spirit inherent in the Agreement will ultimately determine its effectiveness.
Funded Program means any program, campaign, project, or combination thereof, proposed by an applicant for which the trust fund review panel grants an award of funds from the elder and vulnerable victims trust fund.

Examples of Funded Program in a sentence

  • In addition to all other limitations provided in this Agreement, reimbursement for services rendered under certain Funded Programs may be further limited by rules, regulations and procedures applicable only to that Funded Program.

  • Payments are made at applicable rates up to the amounts identified for each Funded Program as shown in Exhibit B and as otherwise may be limited under this Agreement and the attachments thereto.

  • CONTRACTOR shall be responsible for costs that exceed applicable CMAs. In no case shall payments to CONTRACTOR exceed CMAs. In addition to the CMA limitation, in no event shall the maximum reimbursement that will be paid by COUNTY to CONTRACTOR under this Agreement for any Program Amount be more than the amount identified for each Program Amount for each Funded Program, as identified in this Exhibit B, Section III.

  • Notwithstanding any other provision of this Contract, claims entered into the County’s claim processing information system shall be attributed to a specific Funded Program and Subprogram based upon the plan identified by Contractor when submitting the claim into the County’s claims processing information system.

  • To the extent a recipient of services under this Agreement is eligible for coverage under ▇▇▇▇▇-▇▇▇▇▇/Medi-Cal or Medicaid or Medicare or any other Federal or State funded program (“an eligible beneficiary”), CONTRACTOR shall ensure that services provided to eligible beneficiaries are properly identified and claimed to the Funded Program responsible for such services to said eligible beneficiaries.

  • To the extent that CONTRACTOR does not have funds allocated in the Agreement for a Funded Program that pays for services to a particular eligible beneficiary, CONTRACTOR shall, at the first opportunity, refer said eligible beneficiary to another CONTRACTOR or COUNTY facility within the same geographic area to the extent feasible, which has available funds allocated for that Funded Program.

  • Funded Program or Bona fide Reduction or Elimination of Service The District shall comply with standards set in the Education Code Section 45117 when applicable in reducing services of the bargaining unit members.

  • Regardless of any other provision of this Section II, reimbursement to CONTRACTOR shall not exceed the Maximum Contract Amount and shall not exceed the Maximum Program Amount for each Funded Program, as identified in Exhibit B.

  • For the ▇▇▇▇▇- ▇▇▇▇▇/Medi-Cal Funded Program, CONTRACTOR assumes fiscal responsibility for services provided to all individuals who do not have full-scope Medi-Cal or are not Medi-Cal eligible during the term of this Agreement.

  • Complete and mail the Travel Expense Summary Form and/or the Team Funded Program Expense Reimbursement Form (found in Tools and Tips) with original receipts to the WSGC Program Office.
