Accident and Incident Reports. Any employee covered by this Agreement involved in any accident or incident shall immediately report any accident and any physical injury sustained. When required by the Company, and provided the employee is able, the employees covered by this Agreement before going off duty and before starting his/her next shift, shall make out an accident or incident report, in writing, on forms furnished by the Company, and shall turn in all available names and addresses of witnesses to the accident per the Company’s policies and procedures.

Accident and Incident Reports. RRK shall provide the City with electronic participant accident and incident report forms in any instance where accidents or incidents on or near the facility are reported to or witnessed by RRK representatives. If we notice any of the signs of internal injury, NCP will call 911 right away. ● If the incident happens close to naptime and your child falls asleep soon afterward, NCP will check in every 10 minutes while he or she sleeps.
