Purpose of the Disclosure. Agreement This Disclosure Agreement is being executed and delivered by the County and the Dissemination Agent for the benefit of the Holders and Beneficial Owners of each Series of 2010 Bonds and in order to assist the Participating Underwriters in complying with S.E.C. Rule 15c2-12(b)(5).
Purpose of the Disclosure. Contractor agrees to use any data disclosed to it by the State pursuant to this Agreement solely for the purposes of providing the services specified in the Exam Agreement, as well as troubleshooting, maintenance and product-improvement activities related thereto. Contractor shall be a processor and State the controller of any data provided pursuant to this Agreement.
Purpose of the Disclosure. All parties agree to collect and use any data disclosed to it pursuant to this Agreement solely for the purposes of program evaluation, program improvement, accountability, teacher support, and research as required by this agreement and in accordance with state and federal laws and regulations cited above.
Purpose of the Disclosure. 4.1 CHS uses data in partnership with CCDS in the legitimate educational interest of students to improve instruction and administer student aid programs by reviewing student-level data to identify which students need supports, to determine what supports are needed and to initiate or modify those supports in response to data, aligning with the requirements listed in 34 C.F.R. §99.31 of the Family Educational Rights and Privacy Act (FERPA). CHS shall administer the "Efforts to Outcomes" database for the collection of services and outcomes for students within CCDS community partnership schools in order to continually improve student services which meaningfully contribute to student success.
4.2 For purposes of this Agreement, CHS shall function as an agent of the CCDS only with regard to accessing and maintaining pupil record information necessary for CHS's performance. CHS agrees to the following conditions, as required by 00 X.X.X. §00000 and 34 C.F.R. §99.31(FERPA): (1) CHS is under the direct control of the agency or institution with respect to the use and maintenance of education records; and (2) CHS is subject to the requirements of 34 C.F.R. §99.33(a) governing the use and re-disclosure of personally identifiable information from education records.
4.3 CHS shall comply with all requirements of 34 C.F.R. §99.31(a)(6). CHS shall conduct its analysis in a manner that does not permit personal identification of parents and students by anyone other than representatives of CHS authorized by this Agreement who have a legitimate educational interest in the information for purposes of this Agreement. For the purposes of auditing or evaluating CHS's federal or state supported program, CHS Year shall comply with 34 C.F.R. §§99.31(a) and 99.35.

Purpose of the Disclosure. House Concurrent Resolution No. 126 requests that the LDOE, in consultation with BOR, study certain issues related to educator preparation programs including but not limited to the following: Explore creative options for recruiting high-performing students into the teaching profession; Form close partnerships between school systems and educator preparation programs in order to provide school-based preparation experiences for student teachers, aiming to yield graduates with the pedagogical skills necessary to advance student learning and to meet workforce needs of public schools, especially in critical shortage areas; Analyze educator preparation programs’ admissions requirements and the academic achievement of students in such programs in the effort to determine if there is a correlation between a student’s college grade point average upon program admission and performance on the ACT or Scholastic Aptitude Test and his effectiveness as an educator. To respond to House Concurrent Resolution 126, the parties agree that the data will be used solely for the purposes of conducting the following types of analysis: Calculate the correlation between educational indicators (e.g., GPA, ACT, SAT, GRE, postsecondary degree, time it takes to complete a program, etc.) and educator effectiveness data (value-added and Compass evaluations) by specific pathways, specific years, and overall. Assess whether there is a relationship between a teacher candidate receiving a scholarship (e.g., TOPS scholarship, TEACH grant) and educator effectiveness data (value-added and Compass evaluations). The partners further acknowledge that the data can only be released to authorized officials for the sole purpose as stated above.

The Contractor agrees to collect and use any data disclosed to it pursuant to this Agreement solely for the purposes conducting administrative reviews of all school food authorities participating in the following Child Nutrition Programs (CNP): Child and Adult Care Food Program (CACFP) Summer Food Service Program (SFSP) National School Lunch Program (NSLP) and the Afterschool Snack Program (ASP) School Breakfast Program (SBP) Fresh Fruit and Vegetable Program (FFVP) Seamless Summer Option (SSO)

Contractor agrees to collect and use any data disclosed to it pursuant to this Agreement solely for the purposes of developing and testing a set of micro-credentials intended to provide training to teachers that have the potential to improve science, technology, engineering and math (STEM) teaching and learning across the state.
Purpose of the Disclosure. The Contractor agrees to collect and use any data disclosed to it pursuant to this Agreement solely for the purposes of conducting comparative study and analysis of K-2 classroom observation rubrics.
Purpose of the Disclosure. The Contractor agrees to collect and use any data disclosed to it pursuant to this Agreement solely for the purposes of evaluating the Crescent City Schools (CCS) education model impact on 3rd grade English language arts and math LEAP scores compared to similar students enrolled in non-CCS schools in the New Orleans area and Baton Rouge area.
