Agreement Deliverables. Progress on practice-level data quality and connectivity projects, evidenced by the number of practices that participate in a data quality initiative and/or connect ADT (demographic) or CCD interfaces to the VHIE and/or VXU (immunization) interfaces to the VDH registry
Agreement Deliverables. Full cooperation and participation in required Blueprint for Health program evaluation activities
Agreement Deliverables. Project Manager oversees CHT design and staffing plan as evaluated and approved by the CC leadership team and assists with recruitment and hiring or subcontracting of CHT and MAT staff and any staff funded through an additional service layer initiative.

Agreement Deliverables. The Contractor shall offer at least 10 workshops per year. The State shall not count a workshop as offered unless the Contractor can provide proof of enrollment of the minimum number of registrants required for the specified workshop type as of the start date.
Agreement Deliverables. Program Implementer shall meet the deliverables requirements as set forth in this Attachment A Scope of Work and in Table XX below. (Table XX includes deliverables and due dates identified in the Tasks listed above). ATTACHMENT B: SPECIFIC TERMS AND CONDITIONS FOR THE FOCUS ON ENERGY PROGRAMS
1. Claims Substantiation Program Implementer represents that it has adequately substantiated all claims made as part of the Work according to the requirements of state and federal law. Program Implementer shall substantiate claims made in all Program Materials. Claims include, but are not limited to, statements about the energy efficiency, safety, reliability, or performance of a piece of equipment or category of energy efficiency Measures. For each Program Material submitted to Program Administrator for review and approval, Program Implementer shall identify all claims, and shall submit to Program Administrator written evidence and data to substantiate the claims. If Program Administrator determines that the evidence or data that Program Implementer has submitted to Program Administrator is inadequate to substantiate the claim, then Program Implementer shall either provide additional evidence or data, that in Program Administrator’s discretion is adequate to substantiate the claim, or redraft or eliminate the claim so that the data and evidence, in Program Administrator’s discretion, adequately supports the claims made.
2. Coordination with Other Programs Program Implementer shall coordinate with other energy efficiency programs in Program Implementer’s service territory, including programs offered by other utilities and local governments, as well as programs targeting low-income Customers, where applicable. Coordination shall include enhancing consistency in program offerings to minimize duplicative administrative costs, and enhancing the possibility that programs can be marketed together to avoid duplicative marketing budgets. Program Implementer shall describe in each Monthly Report any notable coordination activities with other energy efficiency programs. During Program implementation, Program Implementer shall not knowingly create market barriers or lost opportunities for other Programs. If Program Administrator determines that Program Implementer’s Program may be creating market barriers or lost opportunities, Program Implementer shall make Program changes to minimize market barriers or lost opportunities.
