An Individual's Right to Privacy Regarding Personal Information. The parties agree that they will comply with all federal and state laws which protect the privacy of the personal information of individuals. It is further agreed that the CCBDD will make reasonable attempts to make available relevant medical and psychological information, relevant work history, and other relevant information as it relates to the provision of service to the employees assigned to provide employment or travel assistance services/supports to individuals. APPENDIX A1: LATERAL TRANSFER REQUEST FORM (COMMUNITY INTEGRATION & EMPLOYMENT SPECIALISTS) In order to use this request form, you must currently hold the classification of Community Integration and Employment Specialist (CIES). The CIES classification is considered “regional.” Please also refer to Article 13A of the bargaining unit agreement for complete details on qualification criteria. A lateral transfer is a transfer to an open position in your current classification at a location in a different region. For purposes of a CIES lateral transfer, all CCBDD sites and CE enclaves fall into two Regions: East or West. If your current work location is East, you can request a transfer to a location that is West and vice versa. If you wish to move within your region, speak with your supervisor and/or manager as an inter-regional transfer is not considered a lateral transfer.   • Access to Personal Information by Subcontractors Supplier agrees to require any subcontractors or agents to which it discloses Personal Information under this Agreement or under any SOW to provide reasonable assurance, evidenced by written contract, that they will comply with the same or substantially similar confidentiality, privacy and security obligations with respect to such Personal Information as apply to Supplier under this Agreement or any SOW. Supplier shall confirm in writing to DXC that such contract is in place as a condition to DXC’s approval of use of a subcontractor in connection with any SOW. Upon request of DXC, Supplier will provide to DXC a copy of the subcontract or an extract of the relevant clauses.   • Passwords and Employee Access Provider shall secure usernames, passwords, and any other means of gaining access to the Services or to Student Data, at a level suggested by Article 4.3 of NIST 800-63-3. Provider shall only provide access to Student Data to employees or contractors that are performing the Services. Employees with access to Student Data shall have signed confidentiality agreements regarding said Student Data. All employees with access to Student Records shall pass criminal background checks.

  • Personal Information Protection Each party represents and warrants that procedures compatible with relevant personal information and data protection laws and regulations will be employed so that processing and transfer of such information and data identifiers will not be impeded. d.

  • Protection of Personal Information Party agrees to comply with all applicable state and federal statutes to assure protection and security of personal information, or of any personally identifiable information (PII), including the Security Breach Notice Act, 9 V.S.A. § 2435, the Social Security Number Protection Act, 9 V.S.A. § 2440, the Document Safe Destruction Act, 9 V.S.A. § 2445 and 45 CFR 155.260.   • Safeguards for Personal Information Supplier agrees to develop, implement, maintain, and use administrative, technical, and physical safeguards, as deemed appropriate by DXC, to preserve the security, integrity and confidentiality of, and to prevent intentional or unintentional non-permitted or violating use or disclosure of, and to protect against unauthorized access to or accidental or unlawful destruction, loss, or alteration of, the Personal Information Processed, created for or received from or on behalf of DXC in connection with the Services, functions or transactions to be provided under or contemplated by this Agreement. Such safeguards shall meet all applicable legal standards (including any encryption requirements imposed by law) and shall meet or exceed accepted security standards in the industry, such as ISO 27001/27002. Supplier agrees to document and keep these safeguards current and shall make the documentation available to DXC upon request. Supplier shall ensure that only Supplier’s employees or representatives who may be required to assist Supplier in meeting its obligations under this Agreement shall have access to the Personal Information.

Pursuant to 42 CFR §431.301 and §431.302, personal information 41 concerning applicants and recipients may be disclosed for purposes directly connected with 42 the administration of this Contract and the State Medicaid Plan.
