Annual Compliance Documentation Required to Be On File with UWNCFL. In addition to the signed Impact Partner Funding Agreement, which is due before the 2013/2014 investment(s) will be dispersed; compliance documentation will be requested in accordance with UWNCFL’s compliance policy. In general, the following compliance documents are required to be updated annually by November 1st. It is strongly recommended that all agencies review UWNCFL’s Compliance Policy to ensure submittal of the appropriate documents with the required components. Agency audited and/or audited consolidated financial statement for each calendar year. Auditor’s management letter/auditor recommendations (or letter from auditor stating that no management letter was issued). Management Letter Response for each calendar year. IRS Form 990 for each calendar year. Annual Report for each year. Annual Board of Directors roster Annual USA Patriot Act Certification Form Annual State Corporations Renewal The following must be provided prior to expiration or if any changes are made IRS determination letter of 501(c)(3) tax-exempt status Agency non-discrimination policy Certificate of Registration with the Florida Dept. of Agriculture Tax Exempt Certificate Delinquent compliance documents will warrant a hold on monthly allocation payments until documents are submitted. Any agency that remains non-compliant for a duration of six (6) months or longer will have Impact Partner status revoked and will not be eligible for funding in the following allocation cycle. In addition, 13/14 investments may be penalized by a percentage not to exceed 20% of the total agency investment for late compliance documentation. Grantee must act to ensure its and its Subcontractors’ compliance with all corrections necessary to address any finding of noncompliance with any law, regulation, audit requirement, or generally accepted accounting principle, or any other deficiency identified in any audit, review, or inspection of the Contract and the services and Deliverables provided.
