Application for Leave of Absence. An application requesting a leave of absence must be submitted to the superintendent not less than two (2) week prior to the time the leave is to commence, provided however, in personal cases exceptions may be granted by the Superintendent. The request for the leave of absence must be in writing setting forth the reasons for the leave of absence and the estimated length of time, and, if requested verification as to the use of the leave of absence must be submitted.
