Applying for Leave. a.) an employee’s application for a leave of absence must be made via telephone to the designated third party administrator (TPA) thirty (30) calendar days in advance of the leave, except in cases of emergency; b.) in cases of emergency, the employee shall contact his/her manager and explain the circumstances requiring emergency leave and must notify the TPA as soon as possible. The leave will either be approved or not approved; the employee must complete the appropriate paperwork within seventy-two (72) hours of the time the leave is approved; c.) the employee’s application must include the beginning and end dates of the leave, with statement of the employee’s intent to return to work;
