ASSESSMENT AND EVALUATION SERVICES. CONTRACTOR shall provide a 15 comprehensive psychosocial assessment and must include an evaluation of the Client’s current problem, 16 substance use history, mental status, community functioning, vocational abilities/impairments, and how 17 the behavioral health barriers impair their participation in employment, employment preparation, school 18 requirements or other WTW activities.

ASSESSMENT AND EVALUATION SERVICES. 24 a) Upon receipt of a referral, or in the case of a walk-in consumer requesting 26 by CONTRACTOR are appropriate to an individual's needs. Assessment is a clinical analysis of the history and current status of the individual's mental or behavioral disorder. Assessment may include 27 relevant cultural issues, history, diagnosis, and the use of testing procedures where appropriate. For a 1 Consumer who is hospitalized, CONTRACTOR shall make a face-to-face assessment prior to the 2 Consumer’s discharge from the facility. When a face-to-face visit in the hospital is not possible, CONTRACTOR shall make an appointment with the Consumer prior to discharge for services to be 3 provided within twenty-four (24) hours after discharge. 4 b) CONTRACTOR shall provide an evaluation of the Consumer's community 5 functioning in several areas including, but not limited to, living situation, daily activities, social support systems, vocational and/or educational activities, and health status. CONTRACTOR shall address 6 cultural issues where appropriate. CONTRACTOR shall document admission of Consumer by 7 completing the entry and evaluation documents of COUNTY’s Management Information System.

BEHAVIORAL HEALTH EDUCATION – CONTRACTOR shall provide a series of 14 classes designed to educate Clients about the relationship between their mental illness and/or substance 15 abuse symptoms and the ability to function. 16 4. INDIVIDUAL, COUPLES AND FAMILY THERAPY – CONTRACTOR shall provide 17 therapeutic interventions consistent with the treatment plan and the Client's vocational goals. 18 CONTRACTOR shall provide such services to Clients either individually, or with the Client’s 19 significant other, or to a Client’s family. 20 5. GROUP THERAPY – CONTRACTOR shall provide therapy to Clients determined 21 appropriate for group services. CONTRACTOR shall establish groups as appropriate to meet the needs 22 of these Clients. Groups for Clients may include, but not be limited to, effect of substance use, 23 recovery, relapse prevention, co-occurring disorders, coping skills, life skills, communication skills, 24 women’s issues, domestic violence, parenting skills/issues and job retention. Parenting classes or 25 groups shall be provided pursuant to the CalWORKs Parenting Curriculum which meets the state 26 Welfare and Institution Code and COUNTY standards. 27 6. CO-OCCURRING DISORDER – CalWORKs Clients referred to CONTRACTOR may 28 have a concurrent mental illness and substance use disorder. The primary diagnosis for these Clients 29 shall not be a factor in CONTRACTOR’s acceptance of these Clients; therefore, CONTRACTOR shall 30 be expected to provide services to Clients with either a primary diagnosis of mental illness or a primary 31 diagnosis of substance use disorder. CONTRACTOR shall provide both rehabilitative and recovery 32 services to such Clients and ensure that such services address the relationship between these two (2) 33 diagnoses. CONTRACTOR may receive referrals for CalWORKs Clients who are thought to be co- 34 occurring, but are then determined not to have a mental health illness. The member receives the services and supports authorized, arranged for and coordinated by the IDT staff; b. The services and supports identified in the MCP as being provided by natural and community supports are being provided; and c. The quality of the services and supports received is adequate and still necessary to continue to meet the needs and preferences of the member and support the member’s outcomes identified in the MCP.

  • Environmental Assessment and Mitigation Development of a transportation project must comply with applicable environmental laws. The party named in article 1, Responsible Parties, under AGREEMENT is responsible for the following: A. The identification and assessment of any environmental problems associated with the development of a local project governed by this Agreement. B. The cost of any environmental problem’s mitigation and remediation. C. Providing any public meetings or public hearings required for the environmental assessment process. Public hearings will not be held prior to the approval of Project schematic. D. The preparation of the NEPA documents required for the environmental clearance of this Project. If the Local Government is responsible for the environmental assessment and mitigation, before the advertisement for bids, the Local Government shall provide to the State written documentation from the appropriate regulatory agency or agencies that all environmental clearances have been obtained.

  • Information Services The Custodian may rely upon information received from issuers of Securities or agents of such issuers, information received from Subcustodians or depositories, information from data reporting services that provide detail on corporate actions and other securities information, and other commercially reasonable industry sources; and, provided the Custodian has acted in accordance with the standard of care set forth in Section 6 (a), the Custodian shall have no liability as a result of relying upon such information sources, including but not limited to errors in any such information.
