BASIC LEASE INFORMATION AND EXHIBITS. The following terms as used herein shall have the meanings provided in this Section 1, unless otherwise addressed by a specific provision of this Lease: Lease Date: June 1, 2021 Address of Tenant/Landlord/Building: 0000 0xx Xxx Xxxx Xxxxxxxxx, XX 00000 Premises: See Exhibit “A” Lease Term: 5 years, beginning on June 1, 2021 (the “Commencement Date”), and ending on May 31, 2026 (the “Termination Date”), with ONE (1) renewal option for a like period as stated herein. This Lease may not be renewed after expiration of the renewal term. Rent: $9.50 per square foot (18,629 square feet x $9.50) = $176,975.50 for the first year; for all subsequent years per, an annual lease adjustment shall be tied to the annual average Montana factored CPI-U (same adjustment method for COL for County employees). Rent shall be paid on a monthly basis. Security Deposit: $2,500.00 Permitted Use: Primary care medical and behavioral health, dental and administrative support services necessary and any other service the federal Health Resources Services Administration (HRSA) permits a federally qualified health center (FQHC) to provide, and for no other use or purpose without Landlord’s prior consent. Tenant’s Responsibilities and Janitorial Services: Tenant responsible for Janitorial Services in the Tenant-dedicated space (Exhibit A), excepting common areas, which Landlord shall maintain. Landlord Responsibilities: Landlord responsible for utility costs (power, gas, water, sewer, etc.); Building maintenance including HVAC, snow removal, common areas, and parking spaces. Insurance: As provided herein. In addition, the Compensation Schedules that are part of Exhibits A and B may be amended, modified and/or replaced by Company in its sole discretion, from time to time, without prior notice.
