Consumer Credit Reports. The Credit Union makes credit available to its members on a regular basis. The Applicant(s)/Account Holder(s) of the account authorizes the Credit Union to obtain consumer reports in connection with credit and business transactions involving the Applicant(s)/Account Holder(s), including but not limited to applying for membership, the opening of a savings or checking account or the issuance of a debit card, or other services provided by the Credit Union, and the Applicant(s)/Account Holder(s) of the account authorize any person, association, or corporation to furnish on request, information concerning the affairs of the Applicant(s)/Account Holder(s). The Applicant(s)/Account Holder(s) also authorizes the Credit Union to furnish information concerning the account to consumer reporting agencies. ELECTRONIC SERVICES AGREEMENT
Consumer Credit Reports. Unless I instructed the Bank not to obtain reports concerning my credit, the Bank may request from credit reporting agencies, reports of my credit history. The Bank’s requests will be consistent with my Agency/ Organization’s agreement with union officials (if applicable). Upon request, the Bank will tell me whether a report was requested, and if so, the name and address of any reporting agency that furnished a report. I understand that the Bank may report Account information to credit bureaus after my account has been canceled. Late payments, missed payments or other defaults on my Account may be reflected in my credit report. If I think the Bank reported erroneous information, I will write the Bank describing the information I think is in error and include a copy of that information. case of cancellation based on clauses (i), (ii) or (iv), my Account is subject to immediate cancellation. In the event of any cancellation, I understand that I must still pay all undisputed amounts due to the Bank under this Agreement. I understand that my Account information may be reported to credit reporting agencies if my Account is canceled. I will surrender the Card upon request to my Agency/Organization. I understand that use of the Card or Account after its cancellation will be considered fraudulent and may cause the Bank to take legal action against me.

In the event that the Applicant(s) is/are individual(s) or is a partnership, the signing of this Agreement shall constitute authorization under the Fair Credit Reporting Act for Xxxxxx to utilize consumer credit reporting agencies to provide reports on said individual(s) or partners in order to permit Xxxxxx to appropriately evaluate the extension of any business credit to the Applicant. Additionally, should any individual(s) guarantee the debt of the Applicant, said individual(s) shall be provided with a copy of this Application, and upon the signing of said guarantee, shall consent to Xxxxxx’x use of consumer credit reporting agency reports to assist in the evaluation of the credit of said xxxxxxxxx(s) for the extension of business credit to the Applicant. There is a $99.00 inspection fee for the required physical on-site inspection which will be charged to the client’s credit card on file. Consumer Credit Reports. Buyer consents Avalon obtaining a consumer or business credit reports on Buyer for (a) the extension of credit, or (b) the collection of debt. All Guarantors of Buyer’s obligations to Avalon consent to Avalon obtaining personal consumer credit reports and business consumer credit reports, on them for (a) the extension of credit to Buyer, or (b) the collection of debt from Buyer and/ or Guarantors.

All Guarantors of Buyer’s obligations to National Lumber consent to National Lumber obtaining personal consumer credit reports and business consumer credit reports, on them for (a) the extension of credit to Buyer, or (b) the collection of debt from Buyer and/or Guarantors.
