Data Entry Requirements a. Definition: The number of ECI delivered service hours of PT, OT, SLP, and SST for the month, and other data required to be entered in TKIDS, that are accurately entered into TKIDS by the 20th day of the month following the delivery of the service expressed as a percentage of the total delivered service hours of PT, OT, SLP, and SST entered into TKIDS for the month. b. Calculation methodology: The number of ECI delivered service hours shown on the Service Profile 12 Month Report for each service type (PT, OT, SLP, and SST) for the month is used for the measure. The numerator consists of the sum of all delivered service hours of PT, OT, SLP, and SST entered into TKIDS by the date due. The denominator consists of the sum of all delivered service hours of PT, OT, SLP, and SST entered into TKIDS by the due date plus the sum of all delivered service hours of PT, OT, SLP, and SST entered into TKIDS after the date due. c. Performance target: 100% of data entered by the due date for PT, OT, SLP, and SST.

Data Entry Requirements a. The Contractor must submit a bi-weekly activities report via Survey Monkey for the services and resources provided. Contractors will submit the activities report using this link: xxxxx://xxx.xxxxxxxxxxxx.xxx/r/RPSYK6M. The reporting period for these funds begins July 1, 2020. The Contractor must report on the following, as applicable: 1. Number of families served 2. Number of participating sites 3. Weekly family support coaching 4.   • E-Verify Requirements To the extent applicable under ARIZ. REV. STAT. § 41- 4401, the Contractor and its subcontractors warrant compliance with all federal immigration laws and regulations that relate to their employees and their compliance with the E-verify requirements under ARIZ. REV. STAT. § 23-214(A). Contractor’s or its subcontractor’s failure to comply with such warranty shall be deemed a material breach of this Agreement and may result in the termination of this Agreement by the City.

  • Delivery Requirements Contractor must ensure that the Goods and/or Services are delivered or provided as required by this Contract, the Purchase Order used by Purchaser, and as otherwise mutually agreed in writing between Purchaser and Contractor. The following apply to all deliveries: (a) Contractor shall make all deliveries to the applicable delivery location specified in the Purchase Order. Such deliveries shall occur during Purchaser’s normal work hours and within the time period mutually agreed in writing between Purchaser and Contractor. (b) Contractor shall ship all Goods and/or Services purchased pursuant to this Contract, freight charges prepaid by Contractor, FOB Purchaser’s specified destination with all transportation and handling charges included. Contractor shall bear all risk of loss, damage, or destruction of the Goods and/or Services ordered hereunder that occurs prior to delivery, except loss or damage attributable to Purchaser’s fault or negligence. (c) All packing lists, packages, instruction manuals, correspondence, shipping notices, shipping containers, and other written materials associated with this Contract shall be identified by the Contract number set forth on the cover of this Contract and the applicable Purchaser’s Purchase Order number. Packing lists shall be enclosed with each shipment and clearly identify all contents and any backorders.
