Data Required. 1. The Policyholder shall maintain a record with respect to each Insured Member under this Policy, showing the Insured Member’s name, sex, age or date of birth, Sum Insured, the Effective Date of Coverage, the date insurance terminates or terminated, changes, with dates noted, of classification, nominee details and other pertinent information as may be necessary to carry out the terms of this Policy.
2. Clerical error in keeping the records shall not invalidate the Policy or the insurance coverage of an Insured Member otherwise validly in force nor continue a Policy or the insurance coverage of an Insured Member otherwise validly terminated, but upon the discovery of such error, it shall be rectified at the earliest.
3. The Policyholder shall furnish the Company with all information and proof which the Company may reasonably require with regard to any matters pertaining to the Policy. All documents furnished to the Policyholder by any Insured Member in connection with the Policy, and other records as may have a bearing on the insurance under this Policy, shall be open for inspection by the Company at all reasonable times.
4. Any personal information collected or held by the Company with respect to each Insured Member under this Policy may be held, used and disclosed by the Company to individuals or organizations associated with the Company with regard to matters pertaining to the Insured Member’s coverage.
5. It shall be the responsibility of the Policyholder to ensure that the personal information provided to the Company is accurate. The Policyholder shall indemnify and keep indemnified the Company against any and all losses, costs, expenses, actions, proceedings suffered by the Company as a result of the Policyholder’s failure to carry out the aforesaid.
Data Required. Parameter Units
1. The total gas production from the facility SCF/day
2. The total dry crude oil production and API gravity of the crude produced by the facility bbl/day and ˚ API
3. The total gas production divided by the total dry oil produced. (Gas oil Ratio (GOR)) SCF/bbl
Data Required. On or before January 1 of each year starting on January 1, 2010, Contractor will submit to the County a request for adjustments to the Service Fee. The following information must be provided with the request in order to be considered by the County:
a) The current Service Fee Rate.
b) The Consumer Price Index (CPI) Unadjusted Percent Change from October to October for the twelve months preceding the year in which the Contractor’s rate adjustment is being calculated.
c) The Producer Price Index (PPI) Unadjusted Percent Change from October to October for the twelve months preceding the year in which the Contractor’s rate adjustment is being calculated.
d) The calculation of the adjustments to the Service Fee Rate that is based on the instructions detailed in 3. which follows.
Data Required. A title which includes the name of the subdivision and its location by number of section, township, range and county.
Data Required. The following data is required:-
(a) Income from all sales for each Utility for the relevant financial year in $M (Table 7, Item 7.7 of ESAA document).
(b) Consumption of electricity for each Utility for the relevant financial year in million of kWh (Table 6, Item 6.7, of ESAA document).
(c) The CPI (as herein defined) for the relevant financial year expressed as a percentage.
Data Required. Regression Equation to compute reference prices according to information example furnished at the end of the current month. . Volume and quality (API and %wtS) of crude blend shipment(s) taken out Tumaco Terminal to be exported. . Volume and quality (API and %wtS) of Xxxxxxx Crude delivered in Xxxxxxx terminal, as well as the volume of deliveries in Mary, Miraflor, Toroyaco and Xxxxx xxxxxx. . Value of base price crude blend delilvered in Tumaco, to be exported.
Data Required. (i) The Policyholder shall maintain a record with respect to each Insured Person or Insured Dependant under this Policy, showing the Insured Person’s or Insured Dependant’s name, sex, date of birth, the date insurance became effective, the date insurance terminated, the date of change of classification and such other data as may be necessary to carry out the terms of the Policy.
(ii) The Policyholder shall furnish the Company with all information and proofs, which the Company may reasonably require with regard to any matters pertaining to this Policy. All documents furnished to the Policyholder by any Insured Person or Insured Dependant in connection with the insurance, together with such records as may have a bearing on the insurance under this Policy, shall be open for inspection by the Company at all reasonable times.
Data Required. Surveyor will send the following data on the following schedule to the Managing Director of Procurement Services, Texas Tech University, X.X. Xxx 00000, Xxxxxxx, Xxxxx, 00000.
a. On or before 30 days prior to the expiration of this Agreement: A certified map identified by county, survey, block, section, and permit number, which shows the location of shot holes or station points used in each survey made under this Agreement, the surface evaluation of each survey, the depth of each hole drilled, the water sands encountered, and an estimate of the water, if any, in each hole; and
b. On or before 90 days prior to the expiration of this Agreement: A certified plat, identified by county, survey, block, section and permit number, for each horizon investigated under this permit, which shows the location of each hole and station point used, the corrected or adjusted instrument readings recorded for each station point, and contoured so that it presents correctly, Surveyor’s interpretation of geological conditions of the land.
Data Required. Vendor requires the following Data elements necessary to provide the Vaccines under this Agreement: ☐ First name and last name ☐ Date of Birth ☐ Gender ☐ CPS email addresses ☐ Date of first dose of vaccination ☐Date of second dose of vaccination (if applicable) ☐ Vaccine information (type, lot, batch, etc.)
Data Required. Vendor requires the following Data elements necessary to provide the Vaccines under this Agreement: ● Vaccine information ○ Administered at location/facility name/ID ○ Administered at location type ○ Administration address (including Company) ○ Administration date ○ CVX (product) ○ NDC (national drug code) ○ Dose number ○ Lot number (Unit of Use [UoU] or Unit of Sale [UoS]) ○ MVX (manufacturer) ○ Sending organization (name of the Agency submitting the report) ○ Vaccine administering provider’s name and suffix ○ Administering provider’s address, if different than the administration address ○ Vaccine administration site (on the body) ○ Vaccine expiration date ○ Vaccine route of administration* ○ Vaccine series Data Usage: The data listed above shall be used only for the following purpose: Vendor shall use the above data to schedule vaccine appointments and to track vaccine administration. Vendor shall compile the aforementioned data in the reports detailed in Section 7 of Exhibit A and share those reports with CPS. Via written consent, Vendor shall obtain authorization from each patient to collect this data from patients, and to share the data back with CPS. At all times Vendor agrees to comply with the Administrative Simplification requirements of the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), as set forth in Title 45, Parts 160 and 164 of the Code of Federal Regulations (the "CFR"). In the event of conflicting terms or conditions, this Section shall supersede the other provisions of the Agreement.
