Diabetes Treatment. A. Coverage will be provided for Medically Necessary diabetes treatment and outpatient self-management training and educational services, including medical nutritional counseling at a CareFirst BlueChoice approved facility. Diabetic Supplies are covered under Section 12 herein. Diabetic equipment is covered under Section 11 herein. B. The services must be Medically Necessary as determined by CareFirst BlueChoice for the treatment of diabetes (Types I and II), or elevated blood glucose levels induced by pregnancy. C. In-person, outpatient self-management training and educational services, including medical nutritional therapy, shall be provided through an in-person program provided by an appropriately licensed, registered, or certified CareFirst BlueChoice-approved facility or health care provider whose scope of practice includes diabetes education or management.

Diabetes Treatment. We will provide benefits for equipment, supplies, and outpatient self-management training and education, including medical nutrition therapy, for the treatment of insulin-dependent diabetes, insulin-using diabetes, gestational diabetes, and non-insulin using diabetes if such equipment, supplies and training are prescribed by a licensed, participating health care professional legally authorized to prescribe such items. We will provide benefits for the self- management training and education, including medical nutrition therapy, described above only if provided by a participating certified, registered, or licensed health care professional with specialized training in the education and management of diabetes. You must pay the Cost Share for prescription drugs set forth on your Schedule for diabetic equipment and supplies. Diabetic treatment services are subject to your medical benefit cost share in your Schedule.
Diabetes Treatment. Treatment, education for outpatient self-management, and equipment and supplies are covered when 1. Diabetic daycare self-management and education programs, provided by appropriately licensed participating Health Care Professionals. Training shall include self-management training, education, and medical nutrition therapy necessary to enable Members to properly use the equipment, supplies, and medications prescribed or referred by the Member’s Participating Provider. 2. Diabetic equipment, supplies and medications: a. Insulin, b. Prescriptive medications for the treatment of diabetes, c. Glucagon, d. Blood glucose monitors and blood glucose testing strips, e. Blood glucose monitors designed to assist the visually impaired, f. Insulin pumps and all related necessary supplies, g. Ketone urine testing strips, h. Lancets and lancet puncture devices, i. Pen delivery systems for the administration of insulin, j. Podiatric devices to prevent or treat diabetes-related complications, k. Insulin syringes, l. Visual aids, excluding eyewear, to assist the visually impaired with proper dosing of insulin. If coverage under this EOC includes a Prescription Drug Rider, the medications and supplies listed in the Prescription Drug Rider will be covered under the Prescription Drug Rider. Copayments for these medications and supplies are shown on the Prescription Drug Rider.

Related to Diabetes Treatment

  • Medical Treatment Undersigned understands that the Released Parties do not have medical personnel available at the location of the activities. Undersigned hereby grants the Released Parties permission to administer first aid or to authorize emergency medical treatment, if necessary. Undersigned understands and agrees that any such action by the Released Parties shall be subject to the terms of this agreement and release, including any liability arising from the negligence of the Released Parties when administering first aid or authorizing others to do so. Undersigned understands and agrees that the Released Parties do not assume responsibility for any injury or damage which might arise out of or in connection with such authorized emergency medical treatment.

  • Emergency Medical Treatment I grant the Releasees permission to authorize emergency medical treatment as they deem appropriate, and agree that such action by the Releasees shall be subject to the terms of this Agreement. I understand and agree that the Releasees assume no responsibility for any injury or damage that might result from such emergency medical treatment.

  • Xxx Treatment We have not promised you any particular tax outcome from buying or holding the Note.

  • National Treatment and Most-favoured-nation Treatment (1) Each Contracting Party shall accord to investments of investors of the other Contracting Party, treatment which shall not be less favourable than that accorded either to investments of its own or investments of investors of any third State. (2) In addition, each Contracting Party shall accord to investors of the other Contracting Party, including in respect of returns on their investments, treatment which shall not be less favourable than that accorded to investors of any third State. (3) The provisions of paragraphs (1) and (2) above shall not be construed so as to oblige one Contracting Party to extend to the investors of the other the benefit of any treatment, preference or privilege resulting from: (a) Any existing or future free trade area, customs unions, monetary union or similar international agreement or other forms of regional cooperation to which one of the Contracting Parties is or may become a party, or (b) Any matter pertaining wholly or mainly to taxation.

  • Substance Abuse Treatment Information Substance abuse treatment information shall be maintained in compliance with 42 C.F.R. Part 2 if the Party or subcontractor(s) are Part 2 covered programs, or if substance abuse treatment information is received from a Part 2 covered program by the Party or subcontractor(s).

  • National Treatment In the sectors inscribed in its Schedule, and subject to any conditions and qualifications set out therein, each Party shall accord to services and service suppliers of the other Party treatment no less favourable than that it accords, in like circumstances, to its own services and service suppliers.

  • Treatment The Asset Representations Reviewer agrees to hold and treat Confidential Information given to it under this Agreement in confidence and under the terms and conditions of this Section 4.08, and will implement and maintain safeguards to further assure the confidentiality of the Confidential Information.   • Medication 1. Xxxxxxx’s physician shall prescribe and monitor adequate dosage levels for each Client. 2. Xxxxxxx’s physician shall not impose and/or limit dosage capitations for any prescribed medication for the treatment of opioid use disorder.

  • Confidential Treatment The parties hereto understand that any information or recommendation supplied by the Sub-Adviser in connection with the performance of its obligations hereunder is to be regarded as confidential and for use only by the Investment Manager, the Company or such persons the Investment Manager may designate in connection with the Fund. The parties also understand that any information supplied to the Sub-Adviser in connection with the performance of its obligations hereunder, particularly, but not limited to, any list of securities which may not be bought or sold for the Fund, is to be regarded as confidential and for use only by the Sub-Adviser in connection with its obligation to provide investment advice and other services to the Fund.
