Electronic Enrollment and Disenrollment. To the extent that the Contract Holder has elected to electronically enroll eligible employees or retirees and their eligible dependents ("Eligible Party" or Eligible Parties") in the Plan(s) ("Enroll" or "Enrollment") and to "Disenrollment"), and Aetna has agreed to accept Enrollment and Disenrollment information from Contract Holder through a roster and electronically process such Enrollments and Disenrollments, Contract Holder must meet certain administrative and legal requirements set forth in this section of this Group Agreement. Aetna will electronically Enroll Eligible Parties who have elected Plan(s) coverage ("Electronic Enrollment") and electronically Disenroll Members from the Plan(s) ("Electronic Disenrollment"), provided Contract Holder meets the following requirements: • Uses Aetna enrollment and Disenrollment forms approved by CMS for Electronic Enrollments and Disenrollments ("Aetna Enrollment and Disenrollment Forms"). As permitted under Mandates and this Group Agreement, Contract Holder may permit Eligible Parties to electronically submit an election form to enroll in a Plan(s) ("Online Enrollment Form") to Contract Holder ("Online Election Process"). • Confirms that all Aetna Enrollment and Disenrollment Forms and Online Enrollment Forms contain all data required by CMS, prior to requesting that Aetna process any Electronic Enrollments or Disenrollments. • Maintains and provides access to all original Aetna Enrollment and Disenrollment Forms and Online Enrollment Forms completed by Eligible Parties and Members in accordance with the Records section of this Group Agreement and all Mandates. • Submits Electronic Enrollment and Disenrollments to Aetna timely and accurately in accordance with Mandates, Aetna policies and procedures, and this Group Agreement. • Submits to Aetna all data elements that are required by CMS and Aetna with respect to each Electronic Enrollment and Disenrollment, including, but not limited to, the following data elements: • Name • Permanent AddressMedicare Claim Number (HICN) • End Stage Renal Disease (ESRD) • Gender • Date of Birth • Plan Section • Provider Selection • Provider Selection(if applicable) • Group NumberClass Code • Plan ID • Effective Date Contract Xxxxxx agrees to be bound by all Mandates applicable relating to Electronic Enrollment and Disenrollment.   • Re-enrollment Any eligible employees who wish to join the Sick Leave Bank after their first year of eligibility will contribute two (2) days upon joining. Such membership may only be made during the month of October using the appropriate forms. The two (2) required days of leave shall be donated from their account upon enrollment in the Classified Employee Council (CEC).

  • Open Enrollment There shall be an open enrollment period each enrollment year during which eligible employees may change plans. The District shall establish and announce the dates of such open enrollment period, and shall mail open enrollment materials to employees fourteen or more days before the beginning of the open enrollment period. If an eligible employee requests a change of plan, he or she shall continue to be covered under his or her existing plan until coverage under the new plan can be instituted.

  • Disenrollment Adverse Benefit Determination taken by the Division, or its Agent, to remove a Member's name from the monthly Member Listing report following the Division's receipt and approval of a request for Disenrollment or a determination that the Member is no longer eligible for Enrollment in the Contractor.

  • Enrollment The School shall maintain accurate and complete enrollment data and daily records of student attendance.

  • Special Enrollment Under the circumstances described below, referred to as “qualifying events”, eligible employees and/or eligible dependents may request to enroll in the Plan outside of the initial and annual open enrollment periods, during a special enrollment period.

  • Enrollment Procedures The District shall establish an open enrollment period each year for unit members to participate in the Catastrophic Leave Bank. The enrollment period shall be September 1 through December 1. Once a unit member becomes a participant in the Catastrophic Leave Bank, he/she shall not be required to reenroll each year.

  • Initial Enrollment Upon retirement, each new retiree who is eligible to enroll in plans under the Health Benefits Program shall receive uninterrupted coverage under the plan in which he or she was enrolled as an active employee, provided the employee submits all necessary applications and other required documentation in a timely fashion.

  • Maximum Enrollment The maximum number of students who may be enrolled in the School shall be 550 students, unless the School and District mutually agree to increase this number. This maximum enrollment was determined pursuant to negotiations between the District and the Network for the School and is consistent with facilitating the academic success of students enrolled in the School and facilitating the School’s ability to achieve the other objectives specified in this Contract. If the School wishes to enroll more than the maximum number of students listed above, the Network for the School must submit a written request to the District, in form and substance acceptable to the District, for review and consideration as an amendment to this Contract. The District shall approve any reasonable requests as determined by the District. This maximum enrollment should not exceed the capacity of the School facility. Each year, the School will be asked to affirm an annual maximum enrollment that will be used to determine mid-year enrollment and School Choice numbers.
