Faculty Supervisor. UVU shall designate, for each Intern, a Faculty Supervisor, who shall monitor and evaluate the Intern’s performance during the internship and serve as a liaison between UVU and Provider to better xxxxxx communication, expectations, and cooperative efforts between the parties.
Faculty Supervisor. A. The Faculty Supervisor, with assistance from other Elon faculty, agrees to interact with the Student and the Site Supervisor to ensure a quality educational experience at the Student’s field placement. One conference with the Site Supervisor shall be at mid- term, in person or via Skype/phone, and may involve the Student.
Faculty Supervisor. The faculty supervisor will digitally sign Part A and email it to the Department Chair. Once the form is signed by the faculty supervisor, it cannot be changed by the Chair.
