Fire Investigation Bureau a. The minimum requirement to bid a Fire Prevention Investigator position is the classification of Fire Lieutenant and a current State of Florida Fire Safety Inspector Certification b. Employees assigned full time to the Fire Investigation Bureau who have attained and maintain Fire Prevention Investigator I Certification shall receive 1% above their regular rate of pay. c. Employees assigned full time to the Fire Investigation Bureau who have attained and maintain Fire Prevention Investigator II Certification shall receive an additional 2% (for an accumulative total of 3%) above their regular rate of pay. d. Employees assigned full time to the Fire Investigation Bureau who have attained and maintain Fire Prevention Investigator III Certification shall receive an additional 3% (for an accumulative total of 6%) above their regular rate of pay. e. Employees assigned full time to the Fire Investigation Bureau who have attained and maintain Fire Prevention Investigator IV Certification shall receive an additional 3% (for an accumulative total of 9%) above their regular rate of pay. Miami-Dade County and International Association of Fire Fighters Local 1403 have agreed to document, via this Memorandum of Understanding, the acknowledged practice for placement of firefighting personnel in pay steps upon promotion in the Firefighter series. Section 1. Effective upon ratification by both parties, a Firefighter, Fire Lieutenant, or Fire Captain will at the time of promotion to Fire Lieutenant, Fire Captain, or Chief Fire Officer, be placed in the appropriate promotional pay step as noted in the table below.

Fire Investigation Bureau. The following provisions shall be effective the beginning of the first pay period in October 2004: Fire Prevention Investigator I requirements:  Fire Lieutenant and current State of Florida Certified Fire Inspector.  Successfully conducted over 150 fire origin and cause investigations.  Employees assigned full time to the Fire Investigation Bureau who have attained and maintain Fire Investigator I Certification shall receive 1% above their regular rate of pay. Fire Prevention Investigator II requirements:  Fire Prevention Investigator I  Successfully attained and maintain the National Association of Fire Investigator’s qualification as a Certified Fire Explosion Investigator.  Successfully conducted over 300 fire origin and cause investigations. (This includes 150 fire origin and cause investigations as a Fire Prevention Investigator I).

  • Investigational Services This plan covers certain experimental or investigational services as described in this section. This plan covers clinical trials as required under R.I. General Law § 27-20-60. An approved clinical trial is a phase I, phase II, phase III, or phase IV clinical trial that is being performed to prevent, detect or treat cancer or a life-threatening disease or condition. In order to qualify, the clinical trial must be: • federally funded; • conducted under an investigational new drug application reviewed by the Food and Drug Administration (FDA); or • a drug trial that is exempt from having such an investigational new drug application. To qualify to participate in a clinical trial: • you must be determined to be eligible, according to the trial protocol; • a network provider must have concluded that your participation would be appropriate; and • medical and scientific information must have been provided establishing that your participation in the clinical trial would be appropriate. If a network provider is participating in a clinical trial, and the trial is being conducted in the state in which you reside, you may be required to participate in the trial through the network provider. Coverage under this plan includes routine patient costs for covered healthcare services furnished in connection with participation in a clinical trial. The amount you pay is based on the type of service you receive. Coverage for clinical trials does not include: • the investigational item, device, or service itself; • items or services provided solely to satisfy data collection and that are not used in the direct clinical management; or • a service that is clearly inconsistent with widely accepted standards of care.

  • Site Investigation Developer has made a careful investigation of the Site and is familiar with the requirements of the Contract Documents and has accepted the readily observable, existing conditions of the Site.
