Functions of the Team. The Community Learning Disability Team has many functions. Some of these relate to direct involvement with individual clients, others are more strategic in their nature. ▪ Assessment and Care Management Care Managers will co-ordinate the assessment of need and care planning arrangements for people referred to the team that meet the jointly agreed Eligibility Criteria. Health Professionals will not be care managers but will carry out specialist health assessments and provide support and advice to care managers in order to facilitate an effective care plan. ▪ Carers Assessment and Services Care Managers will offer carers a carers assessment. ▪ Health Case Management Health Professionals will also receive referrals for people that meet the jointly agreed Eligibility Criteria but do not require a care management assessment. The most appropriate health professional will case manage the healthcare assessment and health care interventions that follow. ▪ People with Complex Needs Health Professional and Care Managers will also be required to manage some cases of people with complex needs after the assessment and care management process has been completed. The case will be allocated to a team member, as appropriate, depending upon the level and type of expertise required. As people’s needs change it might be necessary to change the professional managing the case (the aim being to match service user needs to the skills and expertise that exist within the team at all times). ▪ Health Promotion Health professionals will have a health promotion role, working closely with the local health promotion team. ▪ Health Facilitation Health professionals will work with primary care teams, community health colleagues and staff involved in secondary care to facilitate the provision of mainstream health services for people with learning disabilities. ▪ Training and Development The team will enable clients, families, and staff across all sectors to become familiar with how to support people with learning disabilities to have their health care needs met. ▪ Service Planning and Development The team will contribute their knowledge and understanding of local issues and good practice, meeting the needs of people with learning disabilities in Barking and Dagenham to the planning and strategy development process. ▪ Commissioning In most cases, services are commissioned in order to meet individual assessed need.
