Some PPHC locations also offer Milestone Health Assessments (executive physicals) for an additional fee, also to be paid at time of service (Appendix C)

Health Assessments. The State shall ensure that all inmates receive timely medical and mental health assessments. Upon intake, the State shall ensure that a medical professional identifies those persons who have chronic illness. Those persons with chronic illness shall receive a full health assessment within seven (7) days of intake, depending on their physical condition. Persons without chronic illness should receive a full health assessment within fourteen (14) days of intake. The State will ensure that inmates with chronic illnesses will be tracked in a standardized fashion. A readmitted inmate or an inmate transferred from another facility who has received a documented full health assessment within the previous twelve (12) months, and whose receiving screening shows no change in health status, need not receive a new full medical and mental health assessment. For such inmates, medical staff and mental health professionals shall review prior records and update tests and examinations as needed.
Health Assessments. 18.1 In line with the National Health Assessment Standards for Rail Safety Workers an employee is required to undertake a Health Assessment to determine whether the employee is either: 18.1.1 Fit for Duty Unconditional; 18.1.2 Fit for Duty subject to Job Modification; 18.1.3 Fit for Duty Conditional; 18.1.4 Fit for Duty Subject to Review; or 18.1.5 Permanently Unfit for Duty.
Health Assessments. (a) St Xxxx xxx require, at its own cost, an Employee to undergo a health assessment: (i) as a condition of employment; (ii) to evaluate an Employee’s fitness for work; or (iii) to evaluate an Employee’s ability to return to work after an extended period of illness or injury. (b) Where a health assessment is a condition of employment, such assessment must be conducted before the conclusion of the probationary period. (c) The health assessment must be conducted by a healthcare professional who has been nominated by the Employee from a choice of two (2) healthcare professionals where practicable. These will be selected by St Xxxx from the Referral List. (d) The Employee agrees to authorise the healthcare professional to release from the health assessment relevant information to St Xxxx to determine the Employees capacity to perform their role. (e) An Employee may: (i) request a copy of the healthcare professional’s report(s) and St Xxxx shall ensure that any such documentation is provided to the Employee; (ii) consult a healthcare professional of their own choice for a second opinion at their own expense or from the Referral List at St John’s expense; and (iii) appeal a decision made by St Xxxx that is based on information provided by the St Xxxx nominated healthcare professional. Such appeal must be made to the relevant Chief for a review of the decision.
