Furnishing of evidence. An employee who has suffered any injury for which he/she is receiving payment or payments for incapacity in accordance with the provisions of the respective Act shall furnish evidence to the Employer from time to time as required by the Employer of such payment and compliance with this obligation shall be a condition precedent to any entitlement under this clause. Any employee who is receiving or who has received accident pay in respect of any injury shall if required by the Employer or other person on his/her behalf authorise the Employer to obtain any information required by such Employer concerning such injury or compensation payable in respect thereof from the insurance company that is liable to pay compensation to such employee pursuant to the respective Act.
Furnishing of evidence. An employee who receives a payment for incapacity in accordance with the Act will, as a condition of entitlement under this schedule, furnish from time to time to the Trust and as required by the Trust evidence of such payment.

(a) An employee who has suffered an injury for which the employee is receiving payment or payments for incapacity in accordance with the provisions of the Workers Compensation Act shall furnish evidence of such payment to PMS as required by PMS and compliance with this obligation shall be a condition precedent to any entitlement under this Clause.
(b) Any employee who is receiving or who has received accident pay in respect of any injury shall, if required by PMS, authorise PMS to obtain any information required by PMS concerning such injury or compensation payable in respect thereof from the insurance company, or in Victoria, the Accident Compensation Commission or its Agent that is liable to pay compensation to such employee pursuant to the respective Act.

Furnishing of evidence. 19.8.1 An employee who is receiving payment or payments for incapacity under the provisions of the appropriate Act must provide evidence of the payment or payments to the satisfaction of the employer from time to time. The employee must comply with this obligation before the payment of any entitlement under the terms of this clause.
19.8.2 An employee who is receiving or who has received accident pay for an injury must, if required to do so by the employer or other person on the employer's behalf, authorise the employer to obtain any information required concerning the injury or compensation payable for that injury from the insurance company that is liable to pay compensation to the employee under the Act.
Furnishing of evidence. An employee who is receiving or who has received accident pay in respect of any injury will, if required by ACS or other person on ACS’s behalf, authorise ACS to obtain any information concerning such injury or compensation.

(b) Any employee who is receiving or who has received accident pay in respect of any injury shall, if required by Tungsten authorise Tungsten to obtain any information required by Tungsten concerning such injury or compensation payable in respect thereof from the insurance company or its Agent that is liable to pay compensation to such employee pursuant to the respective Act.
Furnishing of evidence. Any employee who is receiving or who has received accident pay in respect of any injury will if required authorise the employer to obtain any information concerning the injury or compensation.

An employee who is receiving or who has received accident pay concerning an injury must, if required by the company or someone acting on behalf of the company, authorised the company to obtain information concerning the injury or compensation payable from the insurance company that is liable to pay the compensation to the employee under the Legislation. Any employee who is receiving or who has received accident pay in respect of injury shall if required by the company or other person on his or her behalf authorise the company to obtain any information required by the company concerning such injury or compensation payment in respect thereof from the insurance company that is liable to pay compensation to such employee pursuant to the relevant legislation
