Furnishing of evidence. An employee who has suffered any injury for which he/she is receiving payment or payments for incapacity in accordance with the provisions of the respective Act shall furnish evidence to the Employer from time to time as required by the Employer of such payment and compliance with this obligation shall be a condition precedent to any entitlement under this clause. Any employee who is receiving or who has received accident pay in respect of any injury shall if required by the Employer or other person on his/her behalf authorise the Employer to obtain any information required by such Employer concerning such injury or compensation payable in respect thereof from the insurance company that is liable to pay compensation to such employee pursuant to the respective Act.
Furnishing of evidence. (c) An employee who receives a payment for incapacity in accordance with the Act will, as a condition of entitlement under this schedule, furnish from time to time to the Trust and as required by the Trust evidence of such payment.
Furnishing of evidence. An employee who is receiving or who has received accident pay in respect of any injury will, if required by ACS or other person on ACS’s behalf, authorise ACS to obtain any information concerning such injury or compensation.
Furnishing of evidence. 19.8.1 An employee who is receiving payment or payments for incapacity under the provisions of the appropriate Act must provide evidence of the payment or payments to the satisfaction of the employer from time to time. The employee must comply with this obligation before the payment of any entitlement under the terms of this clause.
