General Conditions of Coverage. (Short Term) – In order to have the sick leave period reinstated up to 130 days, an employee who has been on sick leave absence must return to work for one (1) full day in the case of an unrelated and different disability cause and for thirty (30) calendar days in the case of the same disability cause. – Medical certificate(s) as per Appendix "A" attached hereto may be required as requested by the Employer. Forms will be available from Supervisors, Payroll or Human Resources. The Employer shall be responsible for payment of completed medical certificate, if required. – Successive absences due to the same or related cause may be considered as one continuous period of disability if the employee has met the qualifying conditions for LTD coverage.

For a period of time not to exceed twenty-four (24) months from the first day of the employee shall be eligible to return to the same position classification if capable of performing the required work.
