Group Accident Insurance Plan. The District shall pay premiums for an Accidental Death and Dismemberment (AD&D) Policy for each eligible employee in an amount equal to the employee's base annual salary. In addition, the District shall pay for an AD&D Policy on an eligible spouse or state registered domestic partner equivalent to 40% of the employee coverage amount and 10% of the employee coverage amount for eligible dependent children of the employee Premium increases for the duration of this Agreement will be borne by the District. At the employee's option s/he may purchase supplementary AD&D coverage as described below: Plans: a. Employee Only Plan Employee Amount: $25,000 increments up to a maximum of $250,000 b. Employee with Spouse/State Registered Domestic Partner Only Employee Amount: $25,000 increments up to a maximum of $250,000 Spouse/State registered domestic partner Amount: 50% of the employee elected coverage amount c. Employee with Spouse/State Registered Domestic Partner and Children d. Employee with Children Only

  • Group Insurance Plan The carriers, coverage, and terms and conditions of participation under the District’s Group Insurance Plan are subject to change in accordance with the applicable provisions of Title I, Division 4, Chapter 10 of the California Government Code (Section 3500 et seq.) (Xxxxxx‐Milias‐Xxxxx Act). a. The District contracts with CalPERS for health plan coverage for all regular and newly hired employees (eligibility to be defined by the “CalPERS health plan”). Booklets on the insurance plans will be available to all participants. b. Employees may choose from the available plans offered by CalPERS. Additional premiums will be borne by the employee through payroll deductions and paid to CalPERS by the District each month; and the additional cost for monthly premiums will be deducted evenly from the first and second payroll period of each month. To the extent allowed by law, the District will attempt to deduct the employee’s premium contribution from pre‐tax dollars.
