HMO Members. Provider agrees that the terms of this Agreement shall continue in full force and effect after the effective date of Provider’s termination (either as an in In-Plan Provider for any HMO(s) or as a Crescent Provider) as to HMO Members who are confined to an inpatient facility at the time of such termination, until such HMO Members are ready for discharge from the facility.

HMO Members. All services must be approved by the member’s participating medical group/IPA. If approved, the claim will be covered at the member’s in-network coverage schedule of benefits. If not approved by the member’s participating medical group/IPA, and services are received at Memorial Care, the claim may be denied as stated in the member’s EOC. Contracted providers and HMO participating medical groups and IPAs that admit patients to Memorial Care have been informed about the contract termination so that Anthem Blue Cross members may be admitted to participating network facilities following the contract’s termination date.

  • Prosthodontics We Cover prosthodontic services as follows:

  • Orthodontics We Cover orthodontics used to help restore oral structures to health and function and to treat serious medical conditions such as: cleft palate and cleft lip; maxillary/mandibular micrognathia (underdeveloped upper or lower jaw); extreme mandibular prognathism; severe asymmetry (craniofacial anomalies); ankylosis of the temporomandibular joint; and other significant skeletal dysplasias.

  • Health Plans A. The health plans offered and benefits provided by those plans shall be those recommended by the JLMBC, approved by the City Council, and administered by the Personnel Department in accordance with LAAC Section 4.

  • Medical Examinations An employee may be required by the Employer, at the request of and at the expense of the Employer, to take a medical examination by a physician of the employee's choice. Employees may be required to take skin tests, x-ray examination, vaccination, inoculation and other immunization (with the exception of a rubella vaccination when the employee is of the opinion that a pregnancy is possible), unless the employee's physician has advised in writing that such a procedure may have an adverse affect on the employee's health.

  • Health Care Operations “Health Care Operations” shall have the same meaning as the term “health care operations” in 45 CFR §164.501.

  • First Aid Attendants a) Designated First Aid Attendants shall receive their job rate of pay plus the Ticket Premium rate. All other employees holding valid First Aid Tickets shall receive a premium of five cents (5¢) per hour over and above their job rate. There shall be no stacking or pyramiding of premiums. b) Where a company is paying a bonus or premium(s) greater than set out above, it shall keep such policy in effect. c) Effective July 1, 1994, premiums for designated First Aid Tickets shall be: Xxxxx 0 - $0.85 per hour Xxxxx 0 - $0.50 per hour

  • Volunteers The use of volunteers to perform bargaining unit work, as covered by this agreement, shall not be expanded beyond the extent of existing practice as of June 1, 1986. The Hospital shall submit to the Union, at three (3) month intervals, the number of volunteers for the current month and the number of hours worked and the duties performed.
