Identification of Select Patients. 22.1.1 Subject to availability of Beds and provisions of this Clause 22.1, the Concessionaire shall provide services to all Select Patients who approach the Hospital seeking medical attention without any discrimination. 22.1.2 Any Patient approaching the Hospital for availing Healthcare Services and seeking to avail Inpatient Services as a Select Patient must either (a) produce proof of registration/eligibility under the Government Health Schemes, or (b) procure from the Authority's designated officer an Authorisation Certificate in accordance with this Clause 22.1. 22.1.3 If the Authority’s representative determines that a Patient is eligible under the applicable Annual Expenditure Budget for Non-Government Scheme Select Patients and/or Applicable Law and/or any other applicable policies and directives, the Authority’s representative shall issue a certificate to the effect that such Patient is a Select Patient for the purpose of availing Inpatient Services (the “Authorisation Certificate”). 22.1.4 Any Patient issued an Authorisation Certificate and availing Inpatient Services will be deemed to be a Non-Government Scheme Select Patient entitled to avail Healthcare Services in accordance with the terms of this Agreement. The Authorisation Certificate shall be conclusive and binding on the Authority for payments to be made to the Concessionaire for treatment of such Non-Government Scheme Select Patient in the Hospital in accordance with this Agreement. 22.1.5 The Authority shall at all times ensure that at least one designated representative of the Authority is stationed and on duty at the Hospital. 22.1.6 Notwithstanding anything to the contrary contained in this Agreement, the Authority shall be entitled to modify the process for identification of Select Patient in accordance with the policies and directives of the State Government, and the Concessionaire shall comply with the modified process notified by the Authority.

Identification of Select Patients. (a) Subject to availability of Beds and provisions of this Clause 24, the Concessionaire shall provide services to all Select Patients who approach the Hospital seeking medical attention without any discrimination. (b) Any Patient approaching the Hospital for availing Healthcare Services and seeking to be treated as a Select Patient must procure from the Authority's designated officer an Authorisation Certificate in accordance with this Clause 24. (c) If the Authority’s representative determines that such Patient is eligible under the applicable Annual Budgetary Allocation Notification for Select Patients and Applicable Law, the Authority’s representative shall issue a certificate to the effect that such Patient is a Select Patient (the “Authorisation Certificate”). Any Patient issued an Authorisation Certificate will be deemed to be a Select Patient entitled to avail Healthcare Services in accordance with the terms of this Agreement. (d) The Authorisation Certificate for a Select Patient shall be conclusive and binding on the Concessionaire for treatment of such Select Patient in the Hospital at the rates specified in Clause 30, and on the Authority for payments to be made to Concessionaire in lieu thereof. (e) The Authority shall at all times ensure that at least one Authority designated representative is stationed and on duty at the Hospital.

  • Identification Cards Identification (“ID”) cards are issued by Us for identification purposes only. Possession of any ID card confers no right to services or benefits under this Contract. To be entitled to such services or benefits, Your Premiums must be paid in full at the time that the services are sought to be received.

  • Identification When performing work on District property, Contractor shall be in appropriate work attire (or uniform, if applicable) at all times. If Contractor does not have a specific uniform, then Contractor shall provide identification tags and/or any other mechanism the District in its sole discretion determines is required to easily identify Contractor. Contractor and its employees shall (i) display on their clothes the above-mentioned identifying information and (ii) carry photo identification and present it to any District personnel upon request. If Contractor cannot produce such identification or if the identification is unacceptable to District, District may provide at its sole discretion, District-produced identification tags to Contractor, costs to be borne by Contractor.

  • Distribution of UDP and TCP queries DNS probes will send UDP or TCP “DNS test” approximating the distribution of these queries.

  • Access to Personal Information by Subcontractors Supplier agrees to require any subcontractors or agents to which it discloses Personal Information under this Agreement or under any SOW to provide reasonable assurance, evidenced by written contract, that they will comply with the same or substantially similar confidentiality, privacy and security obligations with respect to such Personal Information as apply to Supplier under this Agreement or any SOW. Supplier shall confirm in writing to DXC that such contract is in place as a condition to DXC’s approval of use of a subcontractor in connection with any SOW. Upon request of DXC, Supplier will provide to DXC a copy of the subcontract or an extract of the relevant clauses. Supplier shall ensure that any failure on the part of any subcontractor or agent to comply with the Supplier obligations under this Agreement or any SOW shall be grounds to promptly terminate such subcontractor or agent. If during the term of this Agreement or any SOW, DXC determines, in its exclusive discretion, that any Supplier subcontractor or agent cannot comply with the Supplier obligations under this Agreement or with any SOW, then DXC may terminate this Agreement in whole or in part (with respect to any SOW for which such subcontractor or agent is providing services), if not cured by Supplier within the time prescribed in the notice of such deficiency.

  • Access to Personnel File Each employee shall have reasonable access to his/her personnel file for the purpose of reviewing any evaluations or formal disciplinary notations contained therein, in the presence of the Director of Personnel or designate. An employee has the right to request copies of any evaluations in this file.
