Included Records. A copy of the individual faculty member’s records from the Sabbatical Leave Committee, Advancement Committee, and Tenure Review Committee shall be forwarded to Human Resources for entry in the official personnel file of the faculty member.

Included Records. 5.1.1 The record of each fully registered patient at the participating GP Practice will be included in the LLR MIG programme, provided that the patient has not chosen to opt out of this information sharing programme and provided that the record bears a fully traced NHS number. 5.1.2 If a patient leaves a participating GP Practice and registers with a Practice that is not a participant of the LLR MIG programme, then their record will not be included in the MIG irrespective of the patient opt-out status. 5.1.3 If a patient leaves a participating GP Practice and registers with a new Practice which is a participant of the MIG programme, their record will be included as soon as the record is ‘live’ in the new Practice, provided that it bears a fully traced NHS number and provided that an opt-out flag has not been applied. Opt-out status will automatically transfer with the patient record from the leaving Practice to the new Practice.

The recorded accountability for assets shall be compared with the existing assets at reasonable intervals, but not less than annually, and appropriate action shall be taken with respect to any differences.
