Individualized Family Service Plan. (IFSP) is a written plan developed by a multidisciplinary team, including the parent, as defined in Section 6.14, which includes statements of: (a) the child’s present levels of development; (b) with the concurrence of the family, the family’s priorities, resources, and concerns related to enhancing the development of the child; (c) the major outcomes expected; (d) the specific early intervention services necessary to meet the unique needs of the child and family to achieve the outcomes; (e) the frequency, intensity and method of service delivery; (f) the natural environments in which the services will take place; (g) the projected dates of service; (h) the name of the Service Coordinator; and (i) the transition plan. 34 C.F.R. § 303.344.
