Follow-up Testing An employee shall submit to unscheduled follow-up drug and/or alcohol testing if, within the previous 24-month period, the employee voluntarily disclosed drug or alcohol problems, entered into or completed a rehabilitation program for drug or alcohol abuse, failed or refused a preappointment drug test, or was disciplined for violating the provisions of this Agreement and Employer work rules. The Employer may require an employee who is subject to follow-up testing to submit to no more than six unscheduled drug or alcohol tests within any 12 month period.

BEREAVEMENT/TANGIHANGA LEAVE 15.1 The employer shall approve special bereavement leave on pay for an employee to discharge any obligation and/or to pay respects to a deceased person with whom the employee has had a close association. Such obligations may exist because of blood or family ties or because of particular cultural requirements such as attendance at all or part of a Tangihanga (or its equivalent). The length of time off shall be at the discretion of the employer.
Industrial Accident and Illness Leave shall be granted for illness or injury incurred within the course and scope of an employee's assigned duties. The employee who has sustained a job-related injury shall report the injury on an Office approved accident form to the immediate supervisor within twenty-four (24) hours. An employee shall report any illness, in writing, to the immediate supervisor within twenty-four (24) hours of knowledge that the illness is an alleged industrial illness. Christmas/New Year’s Scheduling K-1 Employees will receive at least five (5) consecutive days off at Christmas or New Year's. The Hospital will provide time off from the completion of the night shift the day before the holiday that the Employee is granted (either Christmas Day or New Year's Day). Christmas will include Christmas Eve Day, Christmas Day, and Boxing Day and New Year's will include New Year's Eve Day and New Year's Day. Where an Employee requests not to be scheduled off five (5) consecutive days at Christmas or New Year’s, such an Employee is to put her/his request in writing to the Clinical Manager.
