LOST TIME PAYMENT. Class Members may also submit a claim for lost time related to their efforts undertaken to prevent or mitigate fraud and identity theft following the announcement of the Ransomware Attack. Class Members who elect to submit a Claim that includes a Lost Time Payment may claim up to five (5) hours of lost time at a rate of $25 per hour, for a maximum of $125. Class Member who submit a Lost Time Payment Claim will be required to provide a brief narrative of the nature of the lost time and other mitigation efforts for which payment is being sought.

LOST TIME PAYMENT. Please check this box here if you are electing to seek reimbursement for Lost Time you undertook to prevent or mitigate fraud and identity theft following the announcement of the Data Incident.
