It is the Contractors responsibility to provide the Service Manager a detailed Task Order programme for acceptance within the period stated in the Contract Data. The objective of the Employer’s FFD programme is to provide reasonable assurance that the Contractor’s plant employees will perform their tasks in a reliable and trustworthy manner and are not under the influence of any substance or suffer from any health impairment which in any way adversely affects their ability to safely and competently perform their duties. The FFD programme also gives reasonable assurance that the workforce has been trained and their technical competence has been assessed. The Employer’s FFD process is designed to only allow the Contractor’s employees to perform work if they:  Have valid identification documents;  Have been declared free of drugs and alcohol;  Have been declared healthy, physically able and free of any medical condition that could impair their ability to perform the work they have been appointed for;  Have valid work permits;  Have completed the security background verification process;  Have the qualifications required for the task;  Have the minimum induction training required to work on Affected Property;  Have been declared competent and authorised to perform the work they have been appointed for;  Have received specific training required for the work they will be required to perform; and  Have signed a non-disclosure agreement to protect the Employer’s information, they come in contact with. Information the Contractor’s employee must supply:  Identification document;  Work permit (non SA citizens);  Qualifications;  Curriculum Vitae (CV);  Criminal record history; and  Proof of residential address. Forms that the Contractor’s employee must sign  Pre-placement medical examination;  Baseline questionnaire for audiometry;  Medical declaration;  Security permit application;  Consent to disclose criminal information (if the Employer is performing the criminal check);  SAPS enquiry; and  Non-disclosure agreement (protection of information) Fitness for Duty (FFD) requirements - Procedure 335-68 latest Rev 3 The FFD programme objective is to provide reasonable assurance that all employees who are required to perform work on the Koeberg plant perform their tasks in a reliable and trustworthy manner, are not under the influence of any substance, or suffers from any health impairment which in any way adversely affects their ability to safely and competently perform their duties. It also gives reasonable assurance that employees (Contractor and Eskom) have been trained/made aware and their technical competence/awareness has been assessed. These requirements are derived from applicable legislation, regulations, Nuclear Licensing requirements, world best practices and Eskom requirements applicable to the Operator of a Nuclear Power Plant. The requirements differ depending on contractual stipulations and the specific work that the employee is required to perform. As per OHSA the employee’s fitness for duty and safety remains the responsibility of the [Contractor / Consultant], and not Eskom, unless it is stipulated otherwise in the SHE specifications of the contract. Meeting FFD requirements is entirely the responsibility of the [Contractor / Consultant] and all activities described in procedure 335-68 paragraph 5.1 are performed offsite at the cost of the [Contractor / Consultant] before his/her employees will be registered on the FFD system. After contract award the [Project Manager / Employer’s Representative / Employer’s Agent / Purchaser’s Representative / Engineer / Service Manager] completes an Occupational Health Services Person Job Specification in accordance with KGA- 075 and Procedure 335-68 for each contractor employee, which details the occupational conditions of the work activity on site. The specific details when ticked in the job specification inform the induction training the employee needs to complete. The documentation required and the specific induction training will be indicated on “Appendix A: Application to register for the Koeberg FFD Programme form” (Refer to Procedure 335-68 latest Rev) for each employee. It is the Contractors [Contractor / Consultant] responsibility to provide timeously book his employees, including subcontractor employees, for entry medical examinations prior to entering the Service Manager site. Entry medicals form part of Koeberg’s mandatory site access FFD requirements. The [Contractor / Consultant] ensures that all his employees, including subcontractors, brought to site comply with the FFD process requirements prior, during and on completion of all activities. In particular the [Contractor / Consultant] ensures compliance to the nuclear license requirement that all contractor employees classified as radiation workers on completion of their work period on a detailed nuclear site attends an exit medical examination and receive a final whole body count administered by Koeberg’s Radiation Protection Group. For control purposes the last payment of a contract or Task Order programme for acceptance within is withheld if the period stated tax invoice is not accompanied by written confirmation of completed exit medical examinations and other FFD requirements. The [Contractor / Consultant] and the [Project Manager / Employer’s Representative / Employer’s Agent / Purchaser’s Representative / Engineer / Service Manager] ensure that permit holders that no longer require access to the Site follow the FFD exit procedure.
