Maternity Leave Requests. 1. A request for a maternity leave must be filed with the Director for Human Resources by the end of the fourth (4th) month of pregnancy. A doctor's statement, verifying the pregnancy and indicating the estimated date of delivery, must accompany the request. A tentative date for relief from duty should be agreed to by the Building Administrator, the doctor, and the District. It is also recognized that health factors could change, causing a revision of the relief date. A doctor's statement may be requested to support the change in health.
