Medical Certificates. A medical certificate which states any medical limitations/restrictions and prognosis for return to work may be required from an attending physician or an oral surgeon covering illness of more than three (3) consecutive work days and will be necessary for illness of more than five (5) consecutive work days, unless waived by the employer. The maximum period of absence that can be excused by a certificate from an oral surgeon is five (5) consecutive work days for any one (1) period of absence. If the Employer requires the employee to obtain a medical certificate, the employer shall pay the full cost of obtaining the certificate.
