Includes nicotine cessation supplies ordered by a Health Plan physician. Includes medically necessary nutritional supplements ordered by a Health Plan physician. Laboratory Services Covered when ordered by a Health Care Professional including urine drug screens. Radiology Services Covered when ordered by a Health Care Professional. Diagnostic Services Covered when ordered by a Health Care Professional.

Non-Prescription Drugs. Any Officer using legal, over-the-counter non-prescription drugs, who feels in any way impaired shall advise her/his supervisor of such impairment without delay and, whenever possible, prior to reporting for duty. The Department may require that the Officer use accrued sick leave due to the impairment.

Non-Prescription Drugs. Scope of Coverage We will pay for those FDA approved drugs, which require a prescription and are listed in our formulary. Our formulary is available by contacting Xxxxxxx by Xxxxxx Healthcare Member Services Department. We will pay for those non-prescription drugs which are authorized by a professional licensed to write prescriptions and which appear in the Medical drug formulary. We will also pay for medically necessary enteral formulas for the treatment of specifc diseases and for modifed solid food products used in the treatment of certain inherited diseases of amino acid and organic acid metabolism.
Non-Prescription Drugs. 5.8 The organisation has a “zero-tolerance” attitude to employees found to be impaired in the workplace (either Xxxxx’x or it’s customers) through the use of illegal or non-prescription drugs or prescribed medications that have been used contrary to prescription. These drugs include but are not limited to: • Cocaine • Cannabis • Benzodiazepines • Opiates; and • Amphetamines • Analgesics (Pain Killers)
