Optional Critical Illness Insurance the Employer agrees to provide the option for the employee, their spouse and dependent children, to purchase voluntary critical illness insurance ranging from $10,000 to $150,000, one hundred percent ($100%) paid by the employee, in multiples of $5000. Optional Critical Illness Insurance. The University shall pay one hundred percent (100%) of the costs for the plans listed above, except for (d) Optional Life Insurance Plan, (e) Long Term Disability Plan, (f) Optional Accidental Death, Dismemberment and Loss of Sight Plan and (g) Optional Critical Illness Insurance.

Employees who are receiving or who are eligible to receive LTD shall be eligible to participate in the City's Catastrophic Illness Program as set forth in the ordinance governing such program.
