Paid Time Off (PTO). 2 A. DefinitionSection 1: Paid Time Off (PTO) is accrued upon hire or transfer into a benefits-eligible position. Eligible employees must be scheduled to work at least 30 hours per week on a regular basis. PTO is accruals are available for use in the Medical Centerpay period following completion of 90 days of employment. All hours thereafter are available for use in the pay period following the pay period in which they are accrued. Accruals are based upon paid hours up to 2,080 hours per year, excluding overtime. At the time of transition from vacation and medical leave to PTO, employees will have one- half of the hours in their medical leave bank paid out on a one -time basis. The remainder medical leave hours will be placed in a separate account labeled Extended Medical Leave Bank (EMLB). These medical hours will not accrue and will not be paid out at termination. EMLB hours may be accessed following the third consecutive day of absence attributable to the employee 1s illness and/or disability. PTO provides regular status employees with an FTE of 0.50 and greater with an established number of paid hours off per year for personal use. The number of accrued PTO hours per year is intended to include all aspects of time away from 5 work, for example, holidays, vacation, sick, bereavement, medical appointments and personal days. All time off granted whether paid, unpaid, vacation or holiday must be approved by the manager to ensure that the needs of the Hospital are met. Nothing in this article guarantees approval of any time off which shall no longer has been requested. The number of hours earned is proportionate to the number of hours worked and the number of years employed. 11 4. PTO requests above established unit quotas may be accomplished by 12 “shift swaps” following the established guidelines. A senior nurse may 13 request preference, provided that:
14 a. They request such seniority preference in writing, dated, with 15 signature to the Staffing Office not later than five (5) days after 16 being unable to self-schedule PTO and
17 b. They shall not be eligible to exercise such right of seniority if they 18 exercised it during the preceding year.

PTO may be used to off-set a loss incurred during Spring Breaka scheduled vacation for the following: Night, on 20 ThanksgivingPermanent Night, Christmas EveCharge Nurse, Christmasand Nursing Resources Float differentials. It is the nurse’s responsibility to add in the electronic timekeeping system or by exception sheet, New Years Eve, andthe hours to off-set differential loss up to a maximum of twelve (12) hours in a week. Paid Time Off (PTO). 2 A. Definition18.1 Paid Time Off (PTO) benefits are based on full‐time service commencing with the date of hire. Regular part‐time Registered Nurses shall accrue PTO commencing with date of hire on a pro‐rated schedule based on their authorized hours. Temporary and Per Diem Registered Nurses shall receive no benefits. PTO cash out is limited to no more than one (1) time per quarter. Employees must maintain a total of 80 hours in their PTO bank before cash out is approved. The total PTO hours cashed out may not exceed 40 hours per quarter per calendar year unless the maximum accrual amount will be exceeded. Any request for PTO cash out that is outside these stated limits may be granted at the discretion of the Human Resources Director and CFO, if the employee can establish an unforeseen emergency need. A "Request for PTO Cash Out" form must be submitted and approved by the department manager prior to submission to the Human Resource Department. PTO cash outs will be included in the employee's pay check. This policy is to reduce the number of manual checks necessary to process cash out requests. Employees should plan their requests with payroll periods to avoid delay in payment. The cash out form must be submitted to the Human Resources Department prior to the deadline for submittals of timecards or the cash out will not be processed until the following pay period.
18.2 PTO accrues each pay period based upon all hours paid up to a maximum of eighty (80) hours and upon length of service. Eligible Registered Nurses will accrue PTO as follows: PTO accrual includes the former paid holidays: New Year’s Day, Presidents’ Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Day, the Registered Nurse’s birthday, and floating holiday.
18.3 Unused PTO may be carried forward from one year to the next. The maximum benefits a Registered Nurse may have at any time shall equal two (2) year’s PTO at the Registered Nurse’s annual accrual rate. If the Registered Nurse’s earned, but unused, PTO reaches the maximum the Registered Nurse will not accrue any additional benefits. When the Registered Nurse later uses enough PTO to fall below the maximum, he/she will resume earning PTO from that date forward. Registered Nurses do not earn PTO while on leave of absence.
18.4 Paid Time Off (PTO) hours shall be used for vacation, holidays, short‐term illness, family emergencies, religious observances, preventative health care, dental care, personal business, dependent’s school visitation, and other excused elective absences. PTO may be used in a minimum of one (1) hour increments. Approved educational leave, bereavement leave and jury duty are paid in addition to PTO days, and the PTO account is the Medical Center's program of not charged with this time earned by full-time and 3 part-time employees that off.
