Payroll Changes Forms. Employment, salary, job classification, terminations and other related human resource matters shall be established and memorialized by individual Payroll Change forms maintained as a part of each employee’s personnel file. The Human Resources Department shall make available standardized forms which shall be used by all County Offices and Departments in personnel administration, i.e,, FMLA notification and requests, ADA request for accommodation, and related personnel change forms. Any change in an employee’s personnel status or salary shall be made by a Payroll Change form.
