Personal Care Assistance Services. Provide direct support in the home or community to Enrollees in performing tasks they are functionally unable to complete independently due to disability, based on the LTSS Care Plan and/or the self-directed care plan. Services include: • Enrollee assistance with ADLs, such as grooming, personal hygiene, toileting bathing, and dressing • Assistance with monitoring health status and physical condition • Assistance with preparation and eating of meals (not the cost of the meals itself) Service Benefit Detail Reference Coverage Document • Assistance with housekeeping activities (e.g., bed making, dusting, vacuuming, laundry, grocery shopping, cleaning) • Assistance with transferring, ambulation, and use of special mobility devices Assisting the Enrollee by directly providing or arranging transportation (If providing transportation, the personal care assistant must be verified as having a valid driver’s license and liability coverage). Respite Respite can be defined as a service provided to individuals unable to care for themselves that is furnished on a short-term basis because of the absence or need for relief of those persons who normally provide care for the person. Federal financial participation is not claimed for the cost of room and board as respite services are provided in a private home setting, which may be in the person’s home or occasionally in the respite provider’s private residence, depending on Family preference and case- specific circumstances. When an individual is referred to a RI EOHHS-certified respite agency, a respite agency staff person works with the Family to assure they have the requisite information and/or tools to participate and manage the respite services, The Individual/Family will already have an allocation of hours that has been recommended and approved by RI EOHHS. These hours will be released in six (6) month increments. The Individual/Family will determine how they wish to use these hours. Patterns of potential usage might include intermittent or occasional use; routine use of a few hours each week; planned weekends away; a single block of hours that might allow the rest of the Family to spend a few Days together; or some combination of the above.   • Routing for Operator Services and Directory Assistance Traffic For a Verizon Telecommunications Service dial tone line purchased by iNetworks for resale pursuant to the Resale Attachment, upon request by iNetworks, Verizon will establish an arrangement that will permit iNetworks to route the Reseller Customer’s calls for operator and directory assistance services to a provider of operator and directory assistance services selected by iNetworks. Verizon will provide this routing arrangement in accordance with, but only to the extent required by, Applicable Law. Verizon will provide this routing arrangement pursuant to an appropriate written request submitted by iNetworks and a mutually agreed-upon schedule. This routing arrangement will be implemented at iNetworks’ expense, with charges determined on an individual case basis. In addition to charges for initially establishing the routing arrangement, iNetworks will be responsible for ongoing monthly and/or usage charges for the routing arrangement. iNetworks shall arrange, at its own expense, the trunking and other facilities required to transport traffic to iNetworks’ selected provider of operator and directory assistance services.

  • Directory Assistance Service Updates 8.3.3.1 BellSouth shall update end user listings changes daily. These changes include:

  • Ambulance Services Ground Ambulance This plan covers local professional or municipal ground ambulance services when it is medically necessary to use these services, rather than any other form of transportation as required under R.I. General Law § 27-20-55. Examples include but are not limited to the following: • from a hospital to a home, a skilled nursing facility, or a rehabilitation facility after being discharged as an inpatient; • to the closest available hospital emergency room in an emergency situation; or • from a physician’s office to an emergency room. Our allowance for ground ambulance includes the services rendered by an emergency medical technician or paramedic, as well as any drugs, supplies and cardiac monitoring provided. Air and Water Ambulance This plan covers air and water ambulance services when: • the time needed to move a patient by land, or the instability of transportation by land, may threaten a patient’s condition or survival; or • if the proper equipment needed to treat the patient is not available from a ground ambulance. The patient must be transported to the nearest facility where the required services can be performed and the type of physician needed to treat the patient’s condition is available. Our allowance for the air or water ambulance includes the services rendered by an emergency medical technician or paramedic, as well as any drugs, supplies and cardiac monitoring provided.

  • Customer Assistance If any job requires that any aspect of the Services, such as Code implementation, be performed on Customer’s premises, Customer will supply Tealium personnel with such information, resources, and assistance as Tealium may reasonably request. Customer acknowledges and agrees that Tealium’s ability to successfully provide the Services in a timely manner is contingent upon its timely receipt from Customer of such information, resources and assistance as may be reasonably requested by Tealium. Tealium will have no liability for deficiencies or delays in the Services resulting from the acts or omissions of Customer, its agents, or employees or performance of the Services in accordance with Customer’s instructions. Without limiting the foregoing, Customer acknowledges and agrees that if Tealium is unable to deploy all of the Services specified in the Service Order as a result of delay attributable to Customer, then all set-up fees specified in the Service Order will be deemed earned by Tealium and payable by Customer, notwithstanding such delay in deployment.
