Personal Identification. 4.1.6.1. Date of Birth, Preferred Forename, Preferred Surname, In Local Education Authority Care, Legal Forename, Legal Surname, Photo ID.

Personal Identification. If he employer requires employees to carry school issued personal identification, the cost of such personal identification shall be borne by the employer. After a one time replacement, If due to loss or negligence of the employee, the personal identification must be replaced, a reasonable cost may be charged to the employee. Reasonable cost is defined as the unit cost to create the identification.
