Rates and Services. Provider shall provide or arrange for the provision of services in accordance with the Mandates, as and when requested by ILS Community Network or Managed Care Plan from time to time, which may include without limitation the following, as well as the services described by Payor Contract, Waiver application, Mandates, and/or your Provider license. The Provider shall identify to ILS Community Network and the applicable Managed Care Plan if any aspect of Covered Services herein are to be subcontracted by the Provider. Provider will comply with all sections of this Agreement related to Provider subcontracting.

For and in consideration of the sum of charges to be paid by client, THE PAW LIFE agrees that its staff shall provide such loving care as indicated in this contract for the pet(s) listed and owned by client. Minimum Visit Policy while Clients are travelling: THE PAW LIFE has minimum visit policy for the health and well-being of our client’s pet(s). If a Client is away from home for more than 24 hours in a single period (out of town, on holiday, not on premises) The Paw Life would recommend that the Client’s pet(s) must be visited every 12 hours. For pet(s) without access to potty area (i.e. pet door, pee pads, or litter box), THE PAW LIFE requires a minimum of 2 daily visits and strongly recommends 3 daily visits. Visit times are approximate and subject to change based on the needs of all pets under THE PAW LIFE care. THE PAW LIFE is entrusted to use best judgment in caring for pet(s). Other services subject to a supplemental invoice include but are not limited to: Supply Pickup (food, treats, medications etc.), Vet Visit, After Hours Emergency Vet Visit, Extensive Incidental Clean-up, House Emergency (waiting for service person), Return Check/Payment Fee, Pet Sitting Late Payments (payment due at start of service), Dog Walking Late Payments (payment due by date on Invoice). Supplemental invoice is payable in full within 3 days of due date.

In respect of the payment of rates and service charges for essential services and infrastructure, the municipality shall: a) Assist to facilitate community meetings for payment of services; b) Disseminate information regarding payment Capacity and skills development: The Municipality shall: a) provide direct technical assistance, training and capacity building to the Development Committee and other local structures including in respect of: how to run a development Committee effectively (e.g. accountability and reporting back to the Community); developmental processes; council processes; grant funding instruments; b) other training and capacity building in terms of a training plan to be developed collaboratively with the Committee (e.g. in respect of local contactors, community based maintenance etc.).

Rates and Services. (a) Tank Storage Fee. GEL shall pay to Lazarus a rate of $0.50 per shell barrel per month for the use of the Storage Tanks (the "Tank Storage Fee"), for the limited purpose of providing funds to Lazarus for start up of operations at the Facility. Lazarus shall receive the Tank Storage Fee as a part of, and the Tank Storage Fee shall be included in the calculation of the Distribution of Gross Profits (as defined in the Joint Marketing Agreement), until such time as GEL determines, in consultation with Lazarus, that the Tank Storage Fee is no longer necessary to fund Lazarus' operations at the Facility. GEL shall be reimbursed for all Tank Storage Fees paid in accordance with the terms of the Joint Marketing Agreement.
(b) Tank or Pipeline Cleaning. GEL acknowledges that, as of the Effective Date, the Storage Tanks and related pipeline system are acceptable for its use. Upon termination of this Agreement, GEL shall remove its Crude Oil from the Storage Tanks, Facility and pipeline system and will return the Storage Tanks to the same condition they were as of the Effective Date.
