Referrals and Assessment Reports. 24 a. DISTRICT may refer students, as determined by the DISTRICT, with suspected 25 educationally-related mental health needs arising from a qualifying IDEA disability to the COUNTY for 26 mental health assessment within ten days of DISTRICT's receipt of signed consent from the 27 parent/guardian, and the COUNTY will process and complete the assessment within the IDEA timeline.

Referrals and Assessment Reports. COUNTY and LEA agree to abide by the policies and procedures for making student referrals and providing the necessary assessment reports, as provided in Title 2, Division 9, of the California Code of Regulations Section 60040. LEA will refer all students with suspected mental health needs to COUNTY for assessment, and COUNTY will process and complete that assessment as it had before the Governor’s veto on October 8, 2010.
Referrals and Assessment Reports. MHB and the Mendocino County SELPA agree to abide by the policies and procedures for making student referrals and providing the necessary assessment reports as set forth in the IDEA and California Education Code. Assessment Reports including recommendations for level of care, will be presented to the LEA for review five (5) working days before a student’s IEP meeting in which mental health services will be considered. MHB agrees to comply with IDEA assessment timelines (60 calendar days from parent written consent to the assessment to the completion of the assessment with a report written and provided to the LEA). For students with MHB services on the IEPs as of July 1, 2011, MHB agrees to provide the LEA with a summary status report with recommendations for services to the LEA for review five

  • ADDITIONAL REPORTS Upon ADMINISTRATOR’s request, CONTRACTOR shall make such additional reports as required by ADMINISTRATOR concerning CONTRACTOR's activities as they affect the services hereunder. ADMINISTRATOR shall be specific as to the nature of information requested and allow up to thirty (30) calendar days for CONTRACTOR to respond.

  • Progress Reports The Recipient shall submit to the OPWC, at the OPWC's request, summary reports detailing the progress of the Project pursuant to this Agreement and any additional reports containing such information as the OPWC may reasonably require.

  • Adverse Reports The College shall notify an employee in writing of any expression of dissatisfaction concerning his/her work within ten (10) working days of the event of the complaint, with a copy to the Chair of the Union Standing Committee. This notice shall include particulars of the work performance which led to such dissatisfaction. If this procedure is not followed, such expression of dissatisfaction shall not become a part of his/her personnel file for use against him/her at any time. The employee’s reply to such complaint, accusation or expression of dissatisfaction shall become part of his/her personnel file.

  • UNION DUES AND ASSESSMENTS (a) The Employer shall, as a condition of employment, deduct from the wages or salary of each employee in the bargaining unit, whether or not the employee is a member of the Union, the amount of the regular dues payable to the Union by a member of the Union.

  • Final Report The goal of this subtask is to prepare a comprehensive Final Report that describes the original purpose, approach, results, and conclusions of the work performed under this Agreement. The CAM will review the Final Report, which will be due at least two months before the Agreement end date. When creating the Final Report Outline and the Final Report, the Recipient must use the Style Manual provided by the CAM.
