UNION DUES AND ASSESSMENTS. (a) The Employer shall, as a condition of employment, deduct from the wages or salary of each employee in the bargaining unit, whether or not the employee is a member of the Union, the amount of the regular dues payable to the Union by a member of the Union. .
(b) The Employer shall deduct from each employee who is a member of the Union any assessments levied in accordance with the Union's Constitution and Bylaws and/or owing by the employee to the Union.
(c) Deductions shall be made for each biweekly payroll period and membership dues or payments in lieu thereof shall be considered owing in the period for which they are so deducted.
(d) All deductions shall be remitted to the President of the Union no later than the fifteenth (15th) day of the subsequent month after the date of the deduction and the Employer shall also provide the following information by Contract Area: ⇒ Social Insurance Number ⇒ Surname and First Name ⇒ Address ⇒ Job Classification ⇒ Amount of Dues ⇒ Gross Pay
(e) Before the Employer is obliged to deduct any amount under (a) or (b) above, the Union must advise the Employer in writing of the amount of its regular dues. The amount so advised shall continue to be the amount to be deducted until changed by further written notice to the Employer signed by the President of the Union. Upon receipt of such notice, such changed amount shall be the amount deducted.
(f) From the date of the signing of this Agreement and for its duration, no employee organization other than the Union shall be permitted to have membership dues or other moneys deducted by the Employer from the pay of the employees in the bargaining unit.
(g) The Employer shall supply each employee, without charge, a receipt for income tax purposes in the amount of the deductions paid to the Union by the employee in the previous year. Such receipts shall be provided to the employees prior to March 1 of the succeeding year.
(h) An employee shall, as a condition of continued employment, complete an authorization form providing for the deduction from an employee's wages or salary the amount of the regular dues payable to the Union by a member of the Union.
UNION DUES AND ASSESSMENTS. 4.1 On each pay date on which a Member receives a pay cheque the Board shall deduct from each Member the Union Dues chargeable by the Union or the Bargaining Unit. The amount shall be determined by the Union, or the Bargaining Unit as the case may be, in accordance with its constitution and by-laws and made in writing to the Board at least thirty (30) days prior to the expected date of change.
4.2 The Union Dues deducted in clause 4.1 shall be remitted to the Treasurer of the Ontario Secondary School Teachers’ Federation, at the Head Office of the Union, no later than the fifteenth day of the month following the month in which the deductions were made. Such remittance shall be accompanied with the following information on each Member:
(a) Surname and first name (b) Social Insurance Number
UNION DUES AND ASSESSMENTS. L401 On each pay date on which an employee is paid the Board shall deduct from each Occasional Teacher the ETFO dues and any dues chargeable by the Local or an equivalent amount. The amounts shall be determined by ETFO and/or Local in accordance with their respective constitutions and forwarded in writing to the Board at least thirty (30) days prior to the expected date of change. L402 The ETFO dues deducted in L401 shall be remitted to the General Secretary of ETFO no later than the fifteenth of the month following the date on which the deductions were made. Such remittance shall be accompanied by a list showing the names, addresses, wages earned, days worked, wages for the period, amounts deducted, FTE, Ministry Identification Number (MIDENT) and OCT number. The Board shall provide the information in electronic format. L403 Dues specified by the Local in L401, if any, shall be deducted and remitted to the Treasurer of ETFO-LKEOT, no later than the fifteenth of the month following the date on which the deductions were made. Such remittance shall be accompanied by a list identifying the employees, annual salary, the number of days worked, salary for the period, and the amounts deducted. L404 ETFO and/or the Local, as the case may be, shall indemnify and hold the Board harmless from any claims, suits, attachments and any form of liability as a result of such deductions authorized by ETFO and/or the Local.

The Union shall inform the Board, from time to time, of the amount of such dues and assessments.

3.02 The payment shall be accompanied by a dues submission list showing the names, addresses, work sites, wages earned and dues and assessments deducted. In addition to providing a written copy of this information, the Board shall provide the information in electronic form.
UNION DUES AND ASSESSMENTS. 5.01 The Board agrees to deduct from each pay period Federation dues and Local assessments from the wages of each elementary Occasional Teacher.
5.02 The amount of Federation dues and Local assessments to be deducted shall be determined by the Union. The Union shall provide, in writing, thirty (30) days notice to the Board of any change in the monthly deductions.
5.03 The amount of Local assessments to be deducted shall be determined by the Local. The Local shall provide, in writing, thirty (30) days notice to the Board of any change in the monthly deductions.
5.04 Federation dues deducted in accordance with Articles 5.01 and 5.02 shall be forwarded to the General Secretary of E.T.F.O. within thirty (30) days of the dues being deducted. Local assessments deducted in accordance with Articles 5.01, 5.02 and 5.03 shall be forwarded to ETFO, Xxxxxxxx- Xxxxxxxxx Elementary Occasional Teacher Local within thirty (30) days of the dues being deducted.
5.05 The Board shall provide to the Local, by October 1st of each year, a letter stating the total number of days of elementary occasional teaching for the previous school year.
5.06 The Federation dues payment shall be accompanied by a dues submission list showing the names, employee identification number, wages earned, and dues deducted.
5.07 The Federation and Local shall indemnify and save the Board harmless from any claims, suits, attachments and any form of liability as a result of such deductions authorized by the Federation and Local.

4.02 The payment shall be accompanied by a dues submission list showing the names, addresses, worksite location, wages earned and dues and assessments deducted. In addition to providing a written copy of this information, the Board shall, provide the information in electronic form.
UNION DUES AND ASSESSMENTS. A.9.1 The Board shall deduct, on a bi-weekly basis, the Union dues and assessments. Dues and assessments deducted in accordance with this Article shall be forwarded to the General Secretary of the Elementary Teachers’ Federation of Ontario no later than the 15th of the month following the date on which the deductions were made.
UNION DUES AND ASSESSMENTS. 7.1 The Board shall deduct for every pay period and for each member covered by this collective agreement, union dues and assessments. Dues and assessments deducted in accordance with this Article shall be forwarded to the General Secretary of the ETFO within thirty (30) days of the dues being deducted. The Union shall notify the Board from time to time, of the amount of such dues and assessments. Such notice shall be given to the Board, in writing, at least thirty

(d) All deductions shall be remitted to the Union not later than 28 days after the date of deduction and the Employer shall also provide the following information to the Union with every regular dues remittance:
(1) Member SIN XXXXXXXXX, nine digits, no dashes or spaces
(2) Member last name;
(3) Member first name;
(4) Dues XXXX.XX, no commas or dollar signs;
(5) Gross wages for period XXXX.XX, no commas or dollar signs;
(6) Job/position title;
(7) Appointment code: regular, auxiliary, etc.;
(8) Work location name;
(9) Work location address;
(10) Member address;
(11) Member work phone XXXXXXXXXX, 10 digits, no dashes or spaces;
(12) Member home phone XXXXXXXXXX, 10 digits, no dashes or spaces;
(13) Member cell phone XXXXXXXXXX, 10 digits, no dashes or spaces, if the employee has provided it to the Employer;
(14) Member home email, if the employee has provided it to the Employer. The above noted information will be provided electronically in the file formats ".csv". If the Employer is unable to provide the file in ".csv" format then ".xls" or ".xlsx" file formats are acceptable.
(e) The Employer will provide to the Union, on a quarterly basis, a report of employees who have ceased employment and the Record of Employment (XXX) Code used in Block 16 of the XXX form for each of those employees.
(f) Before the Employer is obliged to deduct any amount under (a) or (b) above, the Union must advise the Employer in writing of the amount of its regular dues.
