Rental Assistance Program. (RAP) 20 1. Definition – Provision of financial payments on behalf of a client to a landlord to ensure 21 housing is maintained. 22 2. Scope of ServicesCONTRACTOR shall provide monthly RAP payments on behalf of 23 eligible clients in accordance with U.S. Department of Housing and Urban Development (HUD) 24 requirements for the RAP. 25 3. ELIGIBLITY – CONTRACTOR shall ensure clients meet the following service 26 qualifications for RAP: 27 a. Be homeless or homelessness was documented at initiation of program 28 b. Have an income under 50% of area median income 29 c. Be disabled due to HIV 30 4. UNITS OF SERVICE - A unit is defined as a RAP housing payment. CONTRACTOR 31 shall, at minimum, provide the following units of service: 32 a. Proposed Number of Clients to be Served: 10 33 b. Proposed Units of Service (No. of RAP Housing Payments): 30 34 5. CONTRACTOR shall develop and maintain formal referral relationships with appropriate 35 entities to facilitate early intervention services for low-income individuals with HIV. 36 6.   • Coronavirus Relief Fund – Rental Assistance Application The Coronavirus Relief Fund – Rental Assistance Application should contain all the necessary information to determine whether a Household is potentially eligible for CRF assistance. In accordance with the provisions of Sections 760.20-760.37, Fla. Stat., it is unlawful to discriminate on the basis of race, religion, color, sex, familial status, national origin, or handicap in the award application process for Eligible Housing. 1. At a minimum, an application for program assistance should contain the following items for each household member: a. The number of people residing in the household including name, age, relationship to head of household, current address and home phone number; b. Name and address of employer(s), work phone number(s), and position title with employer; c. Sources of income and a statement signed by all of the adults who reside in the household consenting to the disclosure of information for the purpose of verifying income for determining eligibility for program assistance; d. A signed statement indicating that the applicant understands that all information provided is subject to Florida’s public records laws; and e. A statement that it is a first-degree misdemeanor to falsify information for the purpose of obtaining assistance.

  • Employee Assistance Program Neither the fact of an employee's participation in an employee assistance program, nor information generated by participation in the program, shall be used as a reason for discipline under this Article, except for information relating to an employee's failure to participate in an employee assistance program consistent with the terms to which the employee and the University have agreed.

  • Dependent Care Assistance Program The County offers the option of enrolling in a Dependent Care Assistance Program (DCAP) designed to qualify for tax savings under Section 129 of the Internal Revenue Code, but such savings are not guaranteed. The program allows employees to set aside up to five thousand dollars ($5,000) of annual salary (before taxes) per calendar year to pay for eligible dependent care (child and elder care) expenses. Any unused balance is forfeited and cannot be recovered by the employee.
