Reporting and Certification of Absence. All absences must be reported immediately to the Member’s supervisor, or designate, to the Board’s attendance system and to any site where the Member was scheduled to work. Absences for personal illness or injury for a period not exceeding three (3) consecutive working days may be certified by the supervisor unless the Executive Officer of Human Resources asks specifically for certification in writing by a qualified medical or dental practitioner. A certificate from a qualified medical or dental practitioner may be requested for any absence over three (3) consecutive working days. For absence exceeding one (1) month, the Executive Officer of Human Resources may request an independent medical assessment from a doctor appointed by the Board in order to continue sick leave payments. If the Board asks for a medical certificate to be provided, the Member will be reimbursed for a reasonable cost of providing the required certificate.

Reporting and Certification of Absence. All absences must be reported to the General Secretary immediately. Absences for personal illness or injury for a period not exceeding six (6) days need not be certified unless the General Secretary asks specifically in writing in a particular instance for certification by a qualified medical or dental practitioner. Absence for a period of over three (3) months must be certified by a doctor approved by the Association. Any costs under this article shall be borne by the Association.

  • Notification of Absence A unit member shall contact the office of the division xxxx whenever there is a need to be absent and at least thirty (30) minutes prior to missing any work assignment. Should circumstances prohibit this notification, the unit member shall notify the division office in writing, within one week of returning to work, providing the reasons why advance notification was not given.

  • COMPLIANCE AND CERTIFICATION 25.1 Each Party shall comply at its own expense with all Applicable Laws that relate to that Party’s obligations to the other Party under this Agreement.   • Reporting Absences Employees are responsible for reporting to work on time on each scheduled work day. When an Employee is absent from work, they will notify their Supervisor or designate by telephone before the beginning of the work period or as soon as practicable. The Employee shall inform their Supervisor or designate of the reason for the absence, the expected time of their return to work and a telephone number where they may be reached in their absence.

  • Sick Leave Reporting and Verification Employees must promptly notify their supervisor on their first day of sick leave and each day after, unless there is mutual agreement to do otherwise. If an employee is in a position where a relief replacement is necessary if they are absent, they will notify their supervisor at least two (2) hours prior to their scheduled time to report to work (excluding leave taken in accordance with the Domestic Violence Act). Unless otherwise precluded by law, the Employer has reason to suspect abuse, the Employer may require a written medical certificate for any sick leave absence. An employee returning to work after any sick leave absence may be required to provide written certification from their health care provider that the employee is able to return to work and perform the essential functions of the job with or without reasonable accommodation.

  • Iran Divestment Act Certification The Contractor certifies that, if it submitted a successful bid for this contract, then as of the date it submitted the bid, the Contractor was not identified on the Iran List. If it did not submit a bid for this contract, the Contractor certifies that as of the date that this contract is entered into, the Contractor is not identified on the Iran List. It is a material breach of contract for the Contractor to be identified on the Iran List during the term of this contract or to utilize on this contract any subcontractor that is identified on the Iran List. In this Iran Divestment Act Certification section -- “Contractor” means the person entering into this contract with the City of Durham; and “Iran List” means the Final Divestment List – Iran, the Parent and Subsidiary Guidance– Iran list, and all other lists issued from time to time by the N.C.
