Request for Leave Without Pay. 1. Application for leave without pay must: a. Be submitted in writing in advance; and b. Show the employee’s reason for requesting the leave. 2. In emergency situations, leave without pay may be granted by the chief administrative officer or other designated official without prior application. 3. Eligible employees must provide advance written notice of intent to use leave without pay for FMLA purposes when the need to use the leave is foreseeable. Otherwise, eligible employees must provide such notice as is practicable.
