Requesting Exclusion. Settlement Class Members may elect not to be part of the Class and not to be bound by this Settlement Agreement. Individual requests for exclusion may be submitted to the Settlement Administrator by postal mail. No mass opt-outs are allowed. All requests for exclusion must be in writing and must: (a) clearly identify the case name and number;
Requesting Exclusion. Class Members who want to be excluded from the 19 settlement must send a letter or postcard to the Claims Administrator postmarked no later than 20 one hundred and thirty-five (135) calendar days from when the Court enters this Order stating: (a) 21 the name of the Action, “In re EasySaver Rewards Litigation”; (b) the full name, address, and 22 telephone number of the person requesting exclusion; and (c) a statement that he/she does not 23 wish to participate in the Settlement.
Requesting Exclusion. Class Members who want to be excluded from the 9 Settlement must send a signed letter or postcard to the Claims Administrator stating: (a) the name 10 and case number of the Action; (b) the full name, address, and telephone number of the person 11 requesting exclusion; and (c) a statement that the person does not wish to participate in the 12 Settlement, postmarked no later than seventy-five (75) calendar days after issuance of the Class 13 Notices.
Requesting Exclusion. Class Members who want to be excluded from the 25 Settlement must send a signed letter or postcard to the Claims Administrator stating: (a) the name 26 and case number of the Action; (b) the full name, address, and telephone number of the person 27 requesting exclusion, as well as the email address(es) he or she believes was used to make a 28 1 Qualifying Purchase; and (c) a statement that the person does not wish to participate in the 2 Settlement, postmarked no later than sixty (60) calendar days after issuance of the Class Notices.
Requesting Exclusion. Settlement Class Members may elect to opt out of the Settlement, relinquishing their rights to benefits hereunder. The Notice shall include a procedure for Settlement Class Members to exclude themselves from the Settlement Class by notifying the Settlement Administrator in writing of the intent to exclude himself or herself from the Settlement Class. Settlement Class Members who opt out of the Settlement will not release their claims pursuant to this Agreement. Such written notification must be postmarked no later than the Opt-Out Deadline, as specified in the Notice. The written notification must include the individual’s name and address; a statement that he or she wants to be excluded from the Action; and the individual’s signature. The Settlement Administrator shall provide the Parties with copies of all opt-out notifications, and a final list of all who have timely and validly excluded themselves from the Settlement Class, which Class Counsel may move to file under seal with the Court no later than 10 days prior to the Fairness Hearing. Any Settlement Class Member who does not timely and validly exclude himself or herself shall be bound by the terms of the Settlement.

Requesting Exclusion. “Opt Out.” Any Settlement Class Member who does not wish to participate in this Settlement must submit a Request for Exclusion to the Claims Administrator stating an intention to be “excluded” from this Settlement. The Request for Exclusion must contain the name of the Action, Xxxxxx v. CVS Pharmacy, Inc., Case No. 16-cv-03372, and must also contain the Settlement Class Member's name, current address, and telephone number. The Request for Exclusion must also contain a clear statement of the Class Member’s intent to exclude him or herself, such as “I wish to be excluded from the Class.” The Request for Exclusion must be either (i) personally signed by the Settlement Class Member, dated and mailed to the Claims Administrator and postmarked on or before the Objection/Exclusion Deadline, or (ii) electronically signed by the Settlement Class Member, and submitted to the Claims Administrator through the Settlement Website on or before the Objection/Exclusion Deadline. So-called “mass” or “class” opt-outs shall not be allowed. The date of the postmark on the return mailing envelope and/or the date of online submission through the Settlement Website shall be the exclusive means used to determine whether a Request for Exclusion has been timely submitted. Any Settlement Class Member whose request to be excluded from the Settlement Class is approved by the Court will not be bound by this Settlement Agreement or have any right to object, appeal or comment thereon.
Requesting Exclusion. A Class Member may elect to be excluded from the 2 Settlement Class and to not be bound by the Settlement Agreement. To make this election, a Class 3 Member must mail a written request for exclusion to the Settlement Administrator at the address 4 provided in the Long-Form Notice, postmarked by the Exclusion Deadline ordered by the Court in 5 the Preliminary Approval Order. The request must (a) state the Class Member’s name, address,

A request for exclusion must be submitted no later than ninety (90) calendar days after entry of this Order.
Requesting Exclusion. Class Members who desire to be excluded from the 17 Agreement must mail to Class Counsel the Opt-Out Form attached to the Agreement at Exhibit G. 18 The request for exclusion must be postmarked no later than forty-five (45) calendar days after the
